76例无保护会阴接产的临床研究

摘　　要：目的 通过对无保护会阴接产法在初产妇自然分娩中临床应用效果的探讨,评估其有效性。方法 选取2015年1月~2016年2月期间来本院经阴道自然分娩的初产妇136例,随机分为观察组（76例）和对照组（60例）,对照组采取保护式会阴接产,观察组采取无保护会阴接产法,比较两组会阴损伤情况、各产程所需时间、产后出血、新生儿窒息率以及产后24 h会阴疼痛程度。结果观察组的会阴切开率以及产后出血率均低于对照组（P〈0.05）;而两组在会阴完整率、会阴裂伤率、新生儿窒息率以及在第一、二、三产程所花时间等方面比较无显著差异（P〉0.05）;两组在产后24 h后会阴疼痛程度差异有统计学意义（P〈0.01）。结论 无保护会阴接产法可降低产妇会阴切开率,且产后出血及疼痛率也显著降低,该方法适合在临床上推广使用。Objective To evaluate the effectiveness of clinical application of non-protective perineal delivery in natural childbirth of primip- am.Methods 136 primiparas using vaginal delivery during January 2015-February 2016 in our hospital were choosen as objects,they were randomly divided into observation group（ 76 cases）and control group （60 cases）, Protective perineal delivery were taken by the control group while non-protective perineal delivery were taken by the observation group, compared and analysed perineal injury, the labor time required, postpartum hemorrhage, neonatal asphyxia rate and 24 h postpartum perineal pain in two groups.Results In the observation group, the perine- al incision rate and postpartum hemorrhage rate was less than that of the control group （P〈0.05） ;there existed no significant difference in terms of perineal integrity rate, perineal laceration rate, neonatal asphyxia rate and labor time of each period etc;there existed significant differ- ence in the degree of perineal pain 24 hours after birth between two groups（P〈0.01 ）.Conclusion Non-protective perineal delivery can re- duce the rate of maternal perineal incision, and the rate of postpartum hemorrhage and pain can be reduced significantly, it＇s suitable for clini- cal application.
