利多卡因表面麻醉在鼻内镜检查前的应用研究

摘　　要：目的探讨单纯的鼻内镜插管检查是否需要进行常规的表面麻醉剂与收缩剂。方法分析本院耳鼻喉科接受鼻内镜检查的300例门诊患者临床资料,按检查登记的前后顺序进行编号,将其分为A、B、C3组,A组:检查前使用表面麻醉剂与收缩剂,B组:检查前单纯使用收缩剂,C组:检查前不使用表面麻醉剂与收缩剂,各100例,比较3组检查所用时间、患者不适反应VAS评分。结果3组的检查通过率比较,差异无统计学意义(P>0.05)。A组应用利多卡因进行表面麻醉,检查所用时间及咽反射、疼痛等不适反应的评分均明显低于B组和C组,但由于黏膜刺激导致的恶心反应明显高于B组和C组,经比较,差异具有统计学意义(P<0.05)。结论在鼻内镜检查前应用利多卡因进行表面麻醉确实可以缩短检查时间,减少患者的咽反射和疼痛等不适反应评分,但可以增加恶心反应评分,而总体的鼻内镜检查通过率与检查前不用表面麻醉剂与收缩剂的组别比较,差异无统计学意义(P>0.05)。Objective To explore the simple nasalendoscopic check Whether the need for conventional surface anestheticintubation and shrinkage agent application. Methods Analysis of our hospitalotolaryngology accept nasal endoscopic examination of the clinical data of 300eases of clinic patients, according to check registration sequence number, itcan be divided into three groups A, B, C, group A ： check before use surfaceanesthetie agent and contraction, group B：check before operation wereshrinkage agent, group C：cheek before use surface anesthetic and shrinkageagent, all the 100 cases, the statistical analysis of three groups of patientsto check the time, patient discomfort reaction of the difference of VAS scores.Results Comparing examination pass rate of three groups of patients ,P〉0. 05, there was no statistically significant difference.Application oflidocaine with surface anesthesia group A patients ,check the time and the gagreflex,pain and discomfort ratings were significantly less than group B andgroup C, but sick due to mucosal stimulation reaction obviously higher than thatof group B and group C, by comparison,P〈0. 05, the difference isstatistically significant.Conclusion The application of lidocaine in front ofthe nasal endoscopy with sur- face anesthesia can shorten the testing time, reduce the patients gag reflex reaction discomfort and pain score, but canincrease the gross score reaction, and nasal endoscopic examination pass rateand examination of the overall and shrinkage agent without surface anestheticis the com- parative and no significant difference,P〉0.05.
