凉血消斑冲剂配合匹多莫德治疗风热伤络型过敏性紫癜性肾炎患儿疗效及对免疫功能的影响

摘　　要：目的探讨凉血消斑冲剂配合匹多莫德治疗风热伤络型过敏性紫癜性肾炎患儿疗效及对免疫功能的影响。方法将68例风热伤络型过敏性紫癜性肾炎患儿随机分为联合组和对照组,每组34例。对照组给予常规西医配合匹多莫德治疗,联合组在对照组基础上给予凉血消斑冲剂治疗。观察2组临床疗效及治疗前后体液免疫和细胞免疫功能变化及不良反应和复发情况。结果联合组总有效率明显高于对照组(P<0.05);治疗后联合组IgA、IgG和IgM水平均显著改善(P均<0.05),对照组IgA和IgG显著改善(P均<0.05),而IgM无明显变化(P>0.05),联合组各指标改善情况均明显优于对照组(P均<0.05);治疗后联合组CD4^+、CD8^+和CD4^+/CD8+均显著升高(P均<0.05),而对照组无明显变化(P均>0.05);2组不良反应发生率比较差异无统计学意义(P>0.05),联合组复发率明显低于对照组(P<0.05)。结论凉血消斑冲剂配合匹多莫德治疗风热伤络型过敏性紫癜性肾炎疗效好,可有效改善机体体液和细胞免疫功能,安全可靠,且复发率低,具有推广价值。Objective It is to explore the effect of Liangxue Xiaoban granule combined with Pidotimod on anaphylactoid purpura nephritis of syndrome type of wind-heat injuring collaterals and immune function in children. Methods 68 children with anaphylactoid purpura nephritis of Fengre shangluo type were randomly divided into combination group and control group,each group had 34 cases. The control group was given normal western medicine Pidotimod,the combination group was added with Liangxue Xiaoban decoction on the basis of treatment in control group. Clinical curative effect,the changes of immune function before and after treatment,side effect and recurrence in both groups were observed and compared. Results The total effective rate in combination group was higher than that in control group（ P〈0. 05）. After treatment,the levels of Ig A,Ig G and Ig M all improved significantly in combination group（ P〈0. 05）,but only Ig A and Ig G were improved significantly in control group（ P〈0. 05）. The improvements of every indexes in combination group were better than that in control group（ P〈0. 05）. After treatment the levels of CD4~＋,CD8~＋,and CD4~＋/ CD8~＋were all improved significantly in combination group（ P〈0. 05）,but no obvious changes of these indexes were found in control group（ P〉0. 05）. There was no significant difference in side effect occurrence rate between the two groups（ P〉0. 05）,but the recurrence rate in combination group was lower than that in control group（ P〈0. 05）. Conclusion Liangxue Xiaoban granule combined with Pidotimod has a good curative effect on anaphylactoid purpura nephritis of Fengre shangluo type,which can effectively improve immune function with good safety and reliability,lower recurrence rate,and is worthy of wide application in clinic.
