复方地龙片对2型糖尿病合并非ST段抬高性急性冠脉综合征患者血清脂肪细胞因子、心肌损伤标记物的影响

摘　　要：目的观察复方地龙片对2型糖尿病合并非ST段抬高性急性冠脉综合征(NSTACS)患者血清脂肪细胞因子、心肌损伤标记物的影响。方法将147例2型糖尿病合并NSTACS患者随机分为观察组74例及对照组73例,对照组给予常规西医治疗,观察组在常规治疗基础上给予复方地龙片治疗,2组均连续治疗14 d。观察2组治疗前后心电图中ST段及血清脂肪细胞因子、心肌损伤标记物变化情况,统计2组治疗期间主要心脏不良事件(MACE)发生情况。结果 2组治疗7 d、14 d后心电图中ST段下移程度及缺血导联数目均明显改善(P均<0.05),血清内脂素、抵抗素、瘦素、心型脂肪酸结合蛋白、肌钙蛋白Ⅰ、肌酸激酶同工酶、NT-pro BNP水平均明显降低(P均<0.05),且观察组上述指标改善情况均明显优于对照组(P均<0.05);观察组治疗期间MACE发生率明显低于对照组(P<0.05)。结论复方地龙片能够降低2型糖尿病合并NSTACS患者血清脂肪细胞因子水平,改善缺血损伤心肌程度,降低MACE发生率,具有较好的心肌保护效应。Objective It is to observe the compound earthworm piece on serum adipocyte factor and myocardial injury markers in the patients with type 2 diabetes mellitus and non ST segment elevations acute coronary syndrome（ NSTACS）.Methods 147 patients with type 2 diabetes mellitus and NSTACS were randomly divided into observation group（ 74 cases） and control group（ 73 cases）,both groups were given conventional western medicine treatment,additionally the observation group was given compound earthworm treatment,both groups were treated for 14 days. The changes of ECG ST segment,serum fat cell factor,myocardial injury markers before and after treatment were observed,major adverse cardiac events（ MACE） during the period of treatment were counted. Results After 7 and 14 days＇ treatment,down degree of ST segment in ECG and the numbers of ischemia lead were all improved significantly（ P〈0. 05）,and the levels ofserum lipid element,resistin,leptin,H-FABP,NT-pro BNP,CK-MB,c Tn I were significantly decreased（ P〈0. 05）,and the decrease and improvements of these indexes in observation group were better than that in the control group（ P〈0. 05）. The incidence of MACE was lower in observation group during treatment than that in the control group（ P〈0. 05）. Conclusion Compound earthworm piece could decrease the levels of serum fat cell factors,improve myocardial injury,reduce MACE incidence in the patients with NSTACS,it has good protecting effects on myocardial injury.
