复肾汤联合低分子肝素钙治疗湿热瘀阻型小儿过敏性紫癜性肾炎疗效观察

摘　　要：目的观察复肾汤联合低分子肝素钙治疗湿热瘀阻型小儿过敏性紫癜性肾炎的疗效及对凝血功能的影响。方法将83例湿热瘀阻型过敏性紫癜性肾炎患儿随机分为研究组41例和对照组42例。2组均给予常规基础治疗,在此基础上对照组给予低分子肝素钙治疗,研究组给予复肾汤联合低分子肝素钙治疗。观察2组治疗前后中医证候积分、尿红细胞和24 h尿蛋白含量及凝血功能指标[凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、D-二聚体(D-D)和血浆纤维蛋白原(FIB)]变化情况,统计2组临床疗效和不良反应发生情况。结果 2组治疗后中医证候积分及尿红细胞、24 h尿蛋白含量、D-D和FIB均显著降低(P均<0.05),PT和APTT均显著升高(P均<0.05),且治疗后研究组各项指标改善情况均明显优于对照组(P均<0.05);研究组总有效率显著高于对照组(P<0.05),不良反应发生率明显低于对照组(P<0.05)。结论复肾汤联合低分子肝素钙治疗湿热瘀阻型小儿过敏性紫癜性肾炎疗效显著,可明显降低尿红细胞、24h尿蛋白含量,改善凝血功能,且不良反应少。Objective It is to observe the curative effect of Fushen decoction combined with low molecular weight heparin on nephritis with anaphylactic purpura of heat-wet stasis syndrome and coagulation function in children. Methods 83 children with anaphylactic purpura of heat-wet stasis syndrome were randomly divided into study group（ n = 41） and control group（ n =42）. Both groups were given normal basic therapy,additionally,the control group was given low molecular weight heparin while the study group was given Fushen decoction combined with low molecular weight heparin. The changes of Chinese medicine syndrome scores,urine RBC,24 h urine protein and coagulation function markers（ PT,APTT,D-D and FIB） before and after treatment in both groups were observed,the clinical curative effects and occurrences of side effects were observed in both groups. Results The Chinese syndrome scores,levels of urine RBC,24 h urine protein,D-D and FIB were significantly decreased,while that of PT and APTT were increased after treatment in both groups（ P〈0. 05）,and the improvements of every index were significantly better in study group than that in control group（ P〈0. 05）. The total effective rate of study group was higher while the occurrence rate of side effect was lower than that in control group（ P〈0. 05）. Conclusion Fushen decoction combined with low molecular weight heparin,which could obviously decrease urine RBC,24 h urine protein,improve coagulation function with less side effect,has good curative effect on nephritis with anaphylactic purpura of heat-wet stasis syndrome in children.
