摘  要:目的:探讨MRI三维动态增强减影技术在浆细胞性乳腺炎中的应用及诊断价值。方法:收集并回顾性分析79例经手术病理证实为浆细胞性乳腺炎患者的临床及影像学资料。入组病例术前均行MRI检查,平扫包括T1WI、T2WI脂肪抑制序列及动态增强减影扫描。所有MRI图像均经2位放射科医生共同分析。结果:31例病灶表现为环形强化,15例表现为分隔状强化,16例表现为结节状强化,9例表现为不规则分叶状强化且边缘可见细小毛刺影,其中有7例窦道或瘘管形成。增强曲线:26例病灶表现为流入型,31例表现为平台型,22例表现为流出型。术前诊断误诊率为39.2%,正确率为60.8%。结论:浆细胞性乳腺炎的MRI表现具有一定特征性,MRI检查有助于明确诊断。Objective To explore the application and diagnostic value of MRI 3D dynamic contrast enhanced subtraction technique for plasma cell mastitis. Methods Totally 79 plasma cell mastitis patients confirmed pathologically underwent retrospective analyses on clinical and imaging data, who went through MRI examinations including plain scan with T1 WI and T2 WI fat suppression sequences as well as dynamic contrast-enhanced subtraction scan. All the MRI images obtained were analyzed by two radiologists jointly. Results There were 31 patients showing ring enhancement, 15 ones having segmental enhancement and 9 ones showing atypical lobular enhancement with tiny spicule shadow at the edge in which there were 7patients with sinus tract or fistula. The enhancement curves were inflow type in 26 patients, platform type in 31 ones and outflow type in 22 ones. The misdiagnosis rate before operation was 39.2%, and the correct rate was 60.8%. Conclusion MRI manifestations of plasma cell mastitis are significant, and are helpful for its final diagnosis.
