X线评估骨质疏松性胸腰椎压缩性骨折经皮椎体成形术疗效  

X-ray evaluation of percutaneous vertebroplasty effects for osteoporotic thoracolumbar vertebral compression fracture

摘　　要：目的:分析X线平片在评估经皮椎体成形术治疗老年骨质疏松性胸腰椎压缩性骨折患者中的应用效果,筛选出客观可行的X线评价指标。方法:选择36例骨质疏松性胸腰椎压缩性骨折患者作为研究对象,均给予经皮椎体成形术,采用椎体形态恢复情况分析椎体形态学变化。用X线平片分别观测伤椎前缘线(A线)、伤椎后缘线(P线)、伤椎中线(M线)、下位椎体中线(NM线)、Cobb角,以P线、NM线作为标准线,计算A线、M线、P线、NM线的比值APP、ANM、MPP、MNM。结果:术后12周,患者A线、P线、M线得到显著提高,变化明显,P<0.05,A线与NM线比值变化更加明显,术后随访,2例患者死亡。结论:经皮椎体成形术有助于恢复老年骨质疏松性胸腰椎压缩性骨折患者椎体前缘高度,以P线、NM线为标准线,计算伤椎前缘线、中线恢复率,可以作为X线评估治疗效果的有效影像学指标。Objective To analyze the efficacy of X-ray plain film in evaluating percutaneous vertebroplasty（PVP） in the treatment of osteoporotic thoracolumbar vertebral compression fracture of the aged patients, and to determine feasible X-ray evaluation indexes. Methods Totally 36 osteoporotic thoracolumbar vertebral compression fracture patients undergoing PVP were enrolled as the subjects, whose changes in vertebral morphology were observed. X-ray plain film was used to observe vertebral leading edge line（A line）, after vertebral edge line（P line）, vertebral midline（M line）, lower vertebral midline（NM line） and Cobb angle. The relative values of A line, M line, P line and NM line, that is, APP, ANM, MPP and MNM, were calculated with P line and NM line as the standard lines. Results A line, P line and M line were increased significantly 12 weeks after operation（P〈0.05）, of which, the relative values of A line and NM line were significant. Postoperative follow-up showed that 2 patients died. Conclusion PVP helps to restore the anterior edge height of the osteoporotic thoracolumbar vertebral compression fracture patient, and the recovery rates of A line and M line calculated with P line and NM line as the standard lines can be effective imaging indexes for PVP X-ray evaluation.
