摘  要:目的:评价镍钛记忆合金加压吻合环(Ni-Ti CAC)在消化道吻合术中的疗效及安全性。方法:检索Pubmed、Embase、CENTRAL、CBM、CNKI和万方等中英文数据库有关Ni-Ti CAC在消化道吻合术中的随机对照试验(RCT),在客观评价文献质量后采用Rev Man 5.0进行Meta分析。结果:共纳入10篇文献1283位患者,Meta分析结果显示:Ni-Ti CAC组在术后排气时间[WMD=-0.12,95%CI(-0.46,0.23),P=0.51]、手术总并发症发生率[RR=0.51,95%CI(0.25,1.01),P=0.05]和术后吻合口瘘发生率[RR=0.68,95%CI(0.20,2.36),P=0.55]方面与对照组效果相当;吻合时间[SMD=-2.99,95%CI(-5.11,-0.88),P<0.05]和术后患者住院时间[WMD=-1.25,95%CI(-2.42,-0.07),P=0.04]方面均优于对照组。结论:Ni-Ti CAC、传统金属钉吻合器及传统手工吻合法应用于消化道吻合术均安全有效,但Ni-Ti CAC较传统方法有一定优势,值得临床推广应用。Objevtive:To evaluate the efficacy and security of nickel-titanium shape memory alloy compression anastomosis clip(Ni-Ti CAC) for gastrointestinal anastomosis.Method:Databases were searched by Pubmed,Embase,CENTRAL,CBM,CNKI and Wangfang for RCT about the treatment of Ni-Ti CAC for gastrointestinal anastomosis.Meta-analysis was analyzed by Rev Man 5.0 software after evaluating the quality of literatures objectively.Result:10 literature and 1283 patients were absorbed into this article,Meta-analysis results showed:there were no differences of Ni-Ti CAC group and control group in total complication rate[RR=0.51, 95%CI(0.25,1.01),P=0.05],flatus time after operation[WMD=-0.12,95%CI(-0.46,0.23),P=0.51] and leakage rate[RR=0.68,95%CI(0.20,2.36),P=0.55].However,the anastomotic time [SMD=-2.99,95%CI (-5.11,-0.88),P〈0.05] and hospital stay time after operation [WMD=-1.25,95%CI(-2.42,-0.07), P=0.04] in the Ni-Ti CAC group were better than the control group.Conclusion:Ni-Ti CAC,traditional metal anastomat and manual anastomosis are effective and secure to the digestive tract anastomosis,but Ni-Ti CAC has certain advantages over the traditional methods,it is worthy of clinical promotion.
