血府逐瘀汤+瓜蒌薤白半夏汤联合西药治疗冠心病心绞痛随机平行对照研究

摘　　要：[目的]观察血府逐瘀汤+瓜蒌薤白半夏汤联合西药治疗冠心病心绞痛疗效。[方法]使用随机平行对照方法,将140例住院患者按掷骰子方法简单随机分为两组。对照组70例单硝酸异山梨酯,20mg/次,2次/d;普伐他汀,20mg/次,1次/d;拜阿司匹林,0.1g/次,1次/d;普萘洛尔,10mg/次,3次/d。治疗组70例血府逐瘀汤+瓜蒌薤白半夏汤:桃仁12g,红花、牛膝、当归、生地黄各9g,赤芍6g,川芎10g,桔梗12g,柴胡10g,瓜蒌30g,薤白18g,半夏12g,茯苓15g,远志10g,太子参30g,甘草10g;胸痛严重加郁金、延胡索;食欲不振加山楂、鸡内金;血液黏稠加当归、决明子、银杏叶;紫绀加三七;眩晕加天麻、钩藤、党参,1剂/d,水煎400m L,早晚分服;西药治疗同对照组。连续治疗20d为1疗程。观测临床症状、心绞痛发作、硝酸甘油用量、心电图、不良反应。治疗1疗程(20d),判定疗效。[结果]治疗组显效47例,有效18例,无效5例,总有效率92.86%;对照组显效31例,有效27例,无效12例,总有效率82.86%;治疗组疗效优于对照组(P<0.05)。[结论]血府逐瘀汤+瓜蒌薤白半夏汤联合西药治疗冠心病心绞痛,疗效满意,无严重不良反应,值得推广。[Objective] To observe the Xuefu Zhuyu Tang Decoction combined with Gualou Xiebai Banxia decoction combined with western medicine in the treatment of coronary heart disease with angina pectoris. [Methods] 140 cases of hospitalized patients were randomly divided into two groups according to the method of random parallel control. The control group of 70 cases of isosorbide mononitrate, 20 mg/times, 2 times/ d; pravastatin, 20 mg/times, 1 times/d; aspirin, 0.1 g/times, 1 times/d; propranolol hydrochloride, 10mg/time, 3 times/d. The treatment group of 70 cases of Xuefu Zhuyu decoction combined with Gualou Xiehai Banxia decoction： Peach 12g, safflower, angelica, achyranthes, radix rehmanniae, Radix Paeoniae Rubra 9g, Chishao 6g, Chuanxiong 10g, Jiegeng 12g, bupleurum 10g, thichosanthes 30g, Allium 18g, pinellia 12g, Poria 15g, Polygala 10g, Radix Pseudostellariae 30g, licorice 10g; severe chest pain and loss of appetite with turmeric, hawthorn, corydalis yanhusuo; Kanekin; blood viscosity with angelica, cassia seed, ginkgo biloba; cyanosis plus 37; vertigo with Gastrodia and Uncaria, Radix Codonopsis, 1 agent/d, Decoction 400mL, sooner or later the service; west ernmedicinewiththecontrolgroup. Continuous treatment for 1 courses of 20d. Observation of clinical symptoms, ECG, adverse reactions. Treatment of 1 courses of treatment （20d）, determine the efficacy. [Results] 47 cases were markedly effective, 18 cases were effective, 5 cases were ineffective, the total effective rate was 92.86%; the control group was 31 cases, 27 cases were effective, 12 cases ineffective, the total effective rate was 82.86%. The treatment group was better than the control group （P〈0.05）. [Conclusion] Xuefu Zhuyu decoction combined with Gualou Xiebai Banxia decoction combined with western medicine in the treatment of angina peetoris of coronary heart disease, the curative effect is satisfied, no serious adverse reactions, worthy of promotion.
