中西医结合治疗股骨头缺血性坏死研究简况

摘　　要：股骨头缺血性坏死(ONFH)是股骨头血运受阻,导致髋关节畸形和功能障碍,与肝肾亏虚、正虚邪侵、气滞血瘀等因素有关,治疗仍是一大难题。影像学检查分4期,对早期股骨头坏死有较高价值。随着中医药、生物材料、人工关节替代、生物力学及显微外科技术发展,诊断治疗取得了显著的发展。治疗方法有中药治疗(补阳还五汤、桃红四物汤、苓桂术甘汤、宣痹汤、左归丸、六味地黄丸等)、针刀,手术(闭合复位经皮空心钉固定术、股骨头髓心减压加自体髂骨打压植骨术、钽棒植入术、全髋关节置换术),中西医结合治疗(钻孔减压术联合中药、He-Ne激光针刀联合药与带血管大转子骨瓣植入联合中药内服)等,中西医结合治疗效果显著,得到广泛认可,未来骨替代材料及辅助生长因子将逐渐被应用。Osteonecrosis of the femoral head（ONFH）is caused by the block of blood transport in femoral head, which results in deformity and dysfunction of the hip joint and is related to the deficiency of liver and kidney, the deficiency of healthy energy and excess of pathogenic factor, and the Qi-stagnancy and blood stasis. And the treatments is still a difficult problem. There are four stages of Imaging studies, which have high value for the early Osteonecrosis of the femoral head. With the development of Chinese medicine, biological materials, Artificial joint replacement, biomechanics and microsurgery, diagnosis and treatment has obtain remarkable development. There are there types of treatments. The first treatment is to use Chinese medicine, such as Buyang huanwu decoction, Taohong Siwu decoction, Linggui zhugan decoction, Xuanbi decoction, Zuogui pill, Liuwei dihuang pill, and to use acupotomy treatment. The second treatment is western surgery, such as Closed reduction and percutaneous screw fixation, Femoral head decompression combined with autogenously iliac bone graft, Implantation of tantalum rod, Total hip arthroplasty. The last treatment is the combination of traditional Chinese medicine and western surgery treatment, such as drilling decompression with Chinese medicine for early treatment, He-Ne laser knife with Chinese medicine, and large vascularized bone graft with taking Chinese medicine. The combination of Chinese medicine treatment and western treatment has have achieved well-known good clinical efficacy, bone substitute material and secondary growth factor will gradually be applied to clinical practice.
