不同直径近段血管对穿支皮瓣成活面积的影响

摘　　要：目的观察SD大鼠穿支皮瓣不同直径的穿支血管、源动脉对皮瓣成活面积的影响。方法将25只健康SD大鼠完全随机分5组，每组5只。A组右侧股动脉未结扎单穿支皮瓣；B组结扎右侧股动脉至股二头肌分支后皮瓣；C组右侧股动脉结扎1／2后穿支皮瓣；D组结扎穿支动脉直径i／2的穿支皮瓣；E组在C组模型基础上继续结扎源动脉至直径的1／4左右。对皮瓣的最大成活面积和皮瓣不同部位灌注量等进行分析。结果A组与B组灌注量差异无统计学意义（P〉0．05），C组与A组、B组差异有统计学意义（P〈0．05）；A组、B组、C组与D组差异有统计学意义（P〈0．05）；C组与D组差异有统计学意义（P〈0．05）；C组与E组差异有统计学意义（P〈0．05）。D组与A组、B组、C组和E组皮瓣成活面积差异有统计学意义（P〈0．05）。结论穿支皮瓣成活面积与是否携带源动脉无关，与皮瓣近段、瓣部血管结构和血流动力学有关。Objective To observe the effects of different diameter perforator blood vessels and source artery on perforator flap survival areas in SD rats. Methods The 25 healthy SD rats were divided into four groups, 5 each group： Group A： the right femoral artery perforator flap animal model~ Group 13： ligation of the right femoral artery to femoral biceps muscle branch flap animal model; Group C： the right femoral artery ligation in 1/2 perforator flap model; Group D： ligation of the perforating artery perforator flap model with diameter of 1/2; Group E： on the basis of C group, the 1/4 of the artery to the diameter was ligated. The flap maximum survival areas and PU values in different parts of the flap were detected. Results For the PU, groups A, B and C had significant difference （P〈0.05） ; compared with group D, there was also significant difference among the groups A, B, C and E （P〈0.05）; significant difference existed between groups C and D （P〈0.05） and between groups C and E （P〈0.05）. For the flap survival areas, compared with group D, there were significant differences among groups A, B, C and E （P〈0.05）. Conclusions The survival areas of perforator flaps with or without perforating artery make no differences, and have a relationship with the vascular structures and hemodynamics of the pedicle and the flap.
