病、证、症结合辨治原发性肝癌

摘　　要：原发性肝癌进展迅速,证型复杂,症状众多,常有辨病、辨证、辨症冲突的情况。临床上应以辨证为主,辨病为根,辨症为辅。辨证选方确定治法方向后结合辨病加减,配伍专方专药,最后针对症状选药。若患者无证可辨或趋于平和体质,则应辨病为先,结合患者影像学结果、实验室检查等,根据肿瘤的形态、代谢、血供、生长情况等进行辨治;若患者病情稳定且证型错杂,但如疼痛、腹泻、腹水或出血等症状严重,影响患者病程发展或生存质量,则应辨症治疗,再结合病证加减。总之,根据扶正与祛邪相结合、辨证与辨病相结合、局部与整体相结合的指导方针,通过病、证、症三位一体结合辨治,在辨证基础上兼顾原发性肝癌的特有规律和发病个体的阶段特征,将现代医学研究成果纳入辨证论治体系,制定中西医综合病、证、症辨治方案,可提高原发性肝癌的治疗效果。The development of primary Liver disease is rapid, and complex syndrome type, many symptoms, often conflict of disease differentiation, syndrome differentiation, and symptom differentiation. Clinical should be based on syndrome differentiation, disease differentiation is the root of the disease, and symptom differentiation supplemented. After determination of therapy, prescription combined with disease differentiation, combined with specific prescriptions, finally choosing medicine for symptoms. If patients without type differentiation or peace constitution, should distinguish disease first, combined with the results of imaging, laboratory examination, according to tumor morphology, metabolism, blood supply, and for growth ; If patients with stable disease and syndrome type variable, but the symptoms such as pain, diarrhea ascites, or serious bleeding and other symptoms, patients with disease pro- gression or seriously affecting the quality of life, symptom differentiation should be treated, combining addition and subtraction dis- ease and syndrome. In conclusion, according to Fuzheng and Quxie combination, disease and syndrome differentiation, the combina- tion of local and global guidelines, through the disease, syndrome, symptom trinity combining differentiation, based on syndrome differentiation in both primary hepatocellular carcinoma specific rules and the onset of the individual stage features, modem medi- cal research results included in the syndromes differentiation system, formulation of integrated traditional Chinese and western med- icine disease, syndrome and symptom syndrome differentiation and treatment, improve the treatment effect of primary liver cancer.
