摘 要:目的 探讨哮喘患儿肺功能的变化及其临床意义。方法 以2014年2月至2015年11月在我院接受治疗的哮喘患儿80例为观察对象,根据其疾病程度分为急性发作期和慢性持续期,同时选取40例健康儿童作为对照组,3组均给予潮式呼吸法检测肺功能。观察3组肺功能指标的变化,比较3组血清炎症细胞因子水平的差异,分析肺功能指标与炎症细胞因子水平及生活质量的相关性。结果 急性发作组患者吸气时间(TI)、呼气时间(TE)和达潮气呼气峰流速(TPTEF)等肺功能指标低于对照组,呼吸频率高于对照组,而3组平均吸气流速(MIF)、平均呼气流速(MEF)和潮气吸气峰流速(PTEF)水平无明显差别;3组患者IL-6、IL-10和肿瘤坏死因子α (TNF-α)等炎症细胞因子水平由高到低分别为急性发作组、慢性持续组和对照组,差异有统计学意义(F =9.689、25.364、42.025、48.952、38.158,P〈0.001);TI/TE 与患者的炎症细胞因子负相关(r = -0.398、-0.412、-0.432、-0.406、-0.382,P〈0.05),与患者生活质量呈正相关(r =0.465,P =0.034)。结论 肺功能检测在哮喘患儿的诊断中具有较好的应用价值,且与患儿的实验室指标密切相关,可作为临床监测指标。Objective To investigate the changes of pulmonary function in children with asthma and its clinical significance. Methods Asthmatic children treated in our hospital from February 2014 to November 2015 accepted treatment were selected as the observation object, according to the extent of the disease, which were divided into acute and chronic persistent period. At the same time, total 40 cases of healthy children as control group, three groups were given the tidal breathing method for detection of pulmonary function. The changes of lung function indexes were observed in three groups. The difference of serum inflammatory cytokines levels between the three groups was compared, and the correlation between the lung function indexes and the levels of inflammatory cytokines and the quality of life were analyzed. Results The acute attack group were inspiratory time (TI), (TE) and expiratory time peak tidal expiratory flow (TPTEF) method of tidal breathing lung function index is lower than the control group, RR was higher than the control group, and three groups of inspiratory flow (MIF), the average expiratory flow rate (MEF) and peak inspiratory tidal velocity (PTEF) levels have no significant difference. The levels of inflammatory cytokines such as interleukin-6 (IL 6), IL-10 and tumor necrosis factor alpha (TNF-a) in the three groups were higher than that in the acute episode group, chronic persistent group and control group, the difference was statistically significant ( F= 9. 689, 25. 364, 42. 025, 48. 952, 38.158, P〈0.001). The lung function indexes were negatively correlated with the inflammatory eytokines in the patients (r =-0.398, -0.412, -0.432, -0.406, -0.382, P 〈0.05), and were postived correlated with life queality ( r = -0.398, -0.412, -0.432, -0.406, -0.382, P〈0.05). Conclusions Pulmonary function test has good application value in the diagnosis of children with asthma,and is closely related to the laboratory indexes of the children.
