摘 要:COPD是一种气道慢性炎症疾病,以不完全可逆的气流受限为特征。研究发现,膈肌损伤在COPD病变进程中发挥重要作用,膈肌纤维类型转变和膈肌纤维萎缩是导致膈肌功能失调的病理基础。膈肌是最主要的呼吸肌,由膈肌损伤引发的膈肌疲劳是COPD患者发生呼吸衰竭的重要因素之一。针对膈肌损伤的有效治疗手段几乎不存在,呼吸肌功能锻炼仅可以在一定程度上改善COPD患者的呼吸功能。COPD膈肌纤维病变的病理生理机制有待进一步研究。Chronic obstructive pulmonary disease (COPD) is a chronic inflammatory airway disease, which is characterized by incomplete reversibility of airflow obstruction. The study found that the diaphragmatic injury plays an important role in the COPD pathological process, and the transformation of diaphragm fiber types and the diaphragm fiber atrophy are the pathological basis of diaphragm dysfunction. Diaphragm is a main respiratory muscle, and the diaphragm fatigue caused by diaphragmatic injury is one of the important factors in COPD patients with respiratory failure. There is almost no effective treatment for the diaphragmatic injury, yet respiratory muscle training can improve the respiratory function in patients with COPD to some extent. The pathological and physiological mechanism of COPD diaphragm fiber lesions needs to be studied further.
