微血管减压术治疗椎动脉相关性面肌痉挛的疗效分析

摘　　要：目的分析微血管减压术(MVD)治疗椎动脉(VA)相关性面肌痉挛的效果及延迟治愈发生情况,同时探讨椎动脉相关性面肌痉挛的手术策略。方法回顾性分析90例面肌痉挛患者的临床资料,依据责任血管的差别分为小血管组和椎动脉相关组,并随访术后疗效及延迟治愈情况。结果两组患者术后有效率无明显差异,椎动脉相关组延迟治愈率明显高于小血管组。结论微血管减压术治疗椎动脉相关性面肌痉挛疗效良好,评估椎动脉相关性面肌痉挛患者术后疗效时应考虑到延迟治愈现象,并适当延长疗效评估的时间。Objective To analyze the clinical effects and delayed resolution of hemifacial spasm（ HFS） associated with the vertebral artery（ VA） after microvascular decompression（ MVD）treatment,and discuss the surgical technique of hemifacial spasm associated with VA. Methods The clinical data of 90 patients with HFS treated by MVD were analyzed retrospectively. Based on the difference of offending vessel,the patients were divided into small vascular group and vertebral artery group,the clinical effects and delayed resolution were recorded. Results There was no significant difference between the two groups in terms of effective rate after MVD,the delayed resolution in the patients with hemifacial spasm after MVD is significantly higher in the vertebral artery group. Conclusion MVD is effective in treating hemifacial spasm associated with VA. The delayed resolution after MVD in patients associated with VA should be considered when evaluating the treatment effect and the follow up time should be prolonged for these patients.
