CT辅助下立体定向穿刺引流术治疗基底节区脑出血的疗效分析

摘　　要：目的探讨CT辅助下立体定向穿刺引流术治疗老年高血压基底节区脑出血的疗效及其并发症。方法将发病24 h内的80例老年高血压脑出血患者分为穿刺引流术组和对照组。两组患者分别采用CT辅助下立体定向穿刺引流术和药物保守治疗;观察比较两组患者的住院治疗时间、并发症、治疗后的神经功能恢复,以及治疗前、后的神经功能评分。结果引流术组患者的并发症发生率为12.5%,对照组则为32.5%。引流术组的住院时间(17.6 d)、最终NIHSS评分(3.28分)和总有效率(92.5%)均明显优于对照组(24.6 d,4.00分和77.5%)。结论 CT辅助下立体定向穿刺引流术治疗基底节区脑出血创伤小,可以明显减少各种并发症,缩短患者的住院时间减少花费。与保守治疗相比,能促进患者神经功能的早期恢复。Objective To analyze the curative effects and complications of CT assisted stereotactic puncture and drainage in the treatment of elderly patients with hypertensive basal ganglia cerebral hemorrhage. Methods 80 cases of elderly patients with cerebral hemorrhage attacking in 24 h were divided into two groups by random allocation method. The research group and the control group were respectively treated with CT assisted stereotactic puncture and drainage or conservative medicine. The hospital stay,complications during hospitalization,neurological function recovery after treatment and neurological function in patients before and after operation in the two groups were observed and compared. Results The total incidence rate of complications in the control group was 32. 5% while in the research group was 12. 5%. The treatment time and final NIHSS score of the research group（ 17. 6 d,3. 28）were better than those of the control group（ 24. 6d,4. 00）. The total effective rate in the research group（ 77. 5%）was better than that in the control group（ 92. 5%）. Conclusions CT assisted stereotactic puncture and drainage can significantly reduce the complications induced by cerebral edema in elderly patients with hypertensive basal ganglia cerebral hemorrhage. It is of good therapeutic effect and the wound is small. Compared with the routinr medicine treatment,it can shorten the hospitalization time and reduce the costs and promote early recovery of nerve function in patients,so it is worth promotion in clinic.
