伊犁州报告为非婚异性性传播的HIV感染者感染原因及真实感染途径复核分析

摘　　要：目的了解伊犁州经非婚异性性途径感染艾滋病病毒（HIV）者的感染情况,以及隐瞒真实感染途径的原因,为当地开展相关流行病学调查工作提供参考依据。方法在当地2014年1-6月抗病毒治疗数据库中,按系统随机抽样原则抽取60名报告为非婚异性性途径感染的HIV感染者,进行一对一、面对面定性访谈,以信息饱和为终止标准,并对感染者的抗病毒治疗管理医生就其感染途径等相关问题进行复核访谈。结果 60名访谈对象,年龄为19~66岁,维吾尔族占78.3%,无业和个体占41.7%;多异性性行为是非婚异性性途径感染HIV的主要原因;与阳性配偶无保护性行为是婚内异性性传播的主要原因;经济利益驱动导致低年龄男性提供男男性行为服务是感染HIV的原因;因访谈对象对HIV传播途径及预防的有效措施不了解而延误诊断时间,以至无法准确判断感染途径是感染途径为不详的原因。访谈对象中,隐瞒真实感染途径的原因主要有：因婚内外多异性性伴而无法准确判断感染途径、社会文化规范对男男性行为者的挤压、害怕因吸毒被强制监管和受到家庭朋友歧视等。结论当地报告因非婚异性性途径感染HIV的人群,其真实感染的原因有多种,应针对不同感染途径人群开展相应有效的干预措施。Objective To know the real reasons of HIV infection among the HIV in fected people who are reported in Yili originally reported as infections through non-married heterosexual contacts,and provide evidence to improve the quality of epidemiology survey.Methods 60 cases were randomly selected from the local ART database during Jan-Jun,2014,who reported their HIV infection through non-married heterosexual contact.An in-depth interview within 30-60 minutes was performed to get the information from the cases.Results The age of all 60 cases in this study ranged from 19 to 66years old,with Uyghurs as a dominant group（78.3%）.41.7% was unemployed or self-employed.The main reasons of the non-married heterosexual HIV infected cases were multiple sex partners.Unsafe intercourse without condom use among the HIV discordant couples was the main reason for the married heterosexual HIV infection.Some young men were infected with HIV through homosexual contacts on commercial purpose.The late diagnosis due to lack of HIV related knowledge caused some patients unaware of their HIV transmission routes.The real reasons of HIV infection were concealed because of their multiple heterosexual partners,social and cultural discrimination against MSM and drug use.Conclusion There are various reasons which cause HIV infection through non-married heterosexual contact.The government should take appropriate actions to know the real causes of HIV infection and have effective HIV/AIDS prevention and control.
