载脂蛋白E基因型检测及积极个体化治疗对冠心病发病率的影响

摘　　要：【目的】研究载脂蛋白E(apolipoprotein,ApoE)基因型检测对冠心病(coronary artery disease,CAD)发病率的影响。【方法】对年龄>65岁排除CAD人群97例采静脉血后分离血清,经不同前处理采用PCR/ASO探针杂交技术进行ApoE基因型检测,观察ApoE基因型与CAD发病率的关系;对ApoE4基因型人群随机分组,追踪5年观察个性化治疗方案干预对CAD发病率的影响研究,其中ApoE4携带者21例、E3携带者52例、E2携带者24例。【结果】(1)与E2携带者相比,E4携带者血总胆固醇、低密度脂蛋白-胆固醇及脂蛋白(a)水平明显增高(P<0.05)。(2)E4携带者CAD发病率为52.3%,明显高于E2携带者(12.5%)(P<0.05)。【结论】apo E基因型检测可预测CAD发病率,可针对易发病人群提供个体化预防治疗方案,预防CAD发病及减少死亡率。[Objective] To study the effect of apolipoprotein E （ApoE） genotype testing on the incidence of coronary artery disease （CAD）. [ Methods ] A total Of 97 cases beyond 65 years old and excluded from CAD were selected. The serum was separated from the venous blood of the cases. After different kinds of pretreatment, ApoE genotype was tested by hybridization technology with PCR/ASO probes. The relationship between ApoE genotype and the incidence of CAD was analyzed. According to the ApoE genotype, the cases were randomly divided into different groups （21 ApoE4 carriers, 52 ApoE3 carriers and 24 ApoE2 carriers）. A 5-year follow-up study was performed to explore the effect of individu- alized intervention on the incidence of CAD. [ Results ] （1） Compared with ApoE2 carriers, blood levels of total cholesterol, low density lipopro- tein cholesterol and lipoprotein （a） in ApoE4 carriers increased significantly （P 〈 0.05）. （2） The CAD incidence of ApoE4 carriers was 52.3%, which was significantly higher than that of ApoE2 carriers （12.5%） （P 〈 0.05）. [Conclusion]The ApoE genotype testing can predict the inci- dence of CAD. According to it, susceptible population can be provided individualized treatment for preventing CAD and reducing the mortality.
