颈动脉重度狭窄脑缺血支架术后CT灌注变化

摘　　要：目的应用CT灌注成像评价单侧颈内动脉(ICA)起始部重度狭窄患者颈动脉支架术(CAS)前后脑灌注的变化。方法选择29例单侧ICA起始部重度狭窄患者,行CT灌注检查,在双侧半球大脑前动脉、大脑中动脉及大脑后动脉供血区选择对称的感兴趣区,测量手术前后相对CT灌注参数:相对平均通过时间(r MTT)、相对达峰时间(r TTP)、相对脑血流量(r CBF)、相对脑血容量(r CBV),进行配对t检验。结果所有患者均成功植入支架,无围手术期并发症,术后狭窄率从术前(82.45±9.26)%改善至(18.35±4.67)%(P<0.01),术后90 d r CBF从术前(0.92±0.03)改善至(0.97±0.02)(P=0.019);r CBV从术前(1.06±0.05)改善至(1.01±0.05)(P=0.047);r MTT从术前(1.12±0.04)改善至(1.04±0.03)(P=0.012);r TTP从术前(1.30±0.10)改善至(1.12±0.04)(P=0.000)。结论 CAS可以改善患者的脑灌注,DSA结合CTP对颈动脉狭窄患者选择手术适应证有指导作用。Objective To evaluate the changes of cerebral perfusion in patients with unilateral internal carotid artery （I- CA） stenosis after carotid artery stenting（CAS） by CT perfusion imaging. Methods A total of 29 cases of unilateral ICA initiation in patients with severe stenosis. The area of interest is selected in the bilateral anterior cerebral artery, middle cerebral artery and posterior cerebral artery underwent CT perfusion examination. Before and after surgery, CT perfusion parameters, relative mean transit time（rMTT）, relative time to peak （rTTP）, relative cerebral blood flow （rCBF）, relative cerebral blood volume （rCBV） , analyzed by paired t test. Results All patients were successfully implanted stents, no perioperative complications, postoperative stenosis rate from preoperative （82.45±9.26）% improved to （18.35±4.67）% （P〈0.01）.90 days after operation, rCBF was improved from （0.92±0.03） to （0.97±0.02） （P=0.019）; rCBV from （1.06±0.05） to （1.01±0.05） （P=0.047）; rMTT from （1.12±0.04） to （1.04±0.03） （P=0.012）; rTTP from （1.30±0.10） to （1.12±0.04） （P= 0.000）. Conclusion CAS can improve the cerebral perfusion, DSA combined with CTP in patients with carotid stenosis have a certain guiding role in the reasonable choice of surgical indications.
