厄贝沙坦联合乐卡地平治疗老年轻中度高血压患者的疗效评价

摘　　要：目的 评价厄贝沙坦联合乐卡地平对老年性高血压的疗效。方法 收集我院2014年6月~2016年3月门诊或入院治疗的老年性轻中度高血压患者116例,随机分为治疗组59例和对照组57例。治疗组予厄贝沙坦（150 mg）联合乐卡地平（10 mg）治疗,qd,对照组予厄贝沙坦（150 mg）联合氢氯噻嗪（12.5 mg）治疗,qd。治疗12周后比较两组血压及脉搏波传导速度（PWV）的数值。结果 12周后两组24 h平均收缩压及舒张压、日间平均收缩压及舒张压、夜间平均收缩压及舒张压较治疗前均有明显降低（P〈0.05）;治疗组夜间收缩压及舒张压下降幅度超过对照组（P〈0.05）。治疗组血压总控制率与对照组比较更高,差异有统计学意义（P〈0.05）。两组的肱踝脉搏波传导速度（ba PWV）及心踝脉搏波传导速度（ha PWV）较治疗前明显减低（P〈0.05）,与对照组比较治疗组的降低幅度更大,差异有统计学意义（P〈0.05）。两组均无发生严重不良反应,对照组少数患者出现血脂、尿酸及电解质的影响。结论 厄贝沙坦联合乐卡地平能有效降低老年性高血压,降压作用持久平稳,同时在血管保护方面更显优势,更有益于减轻靶器官损害,安全性好,推荐用于老年性轻中度高血压联合用药。Objective To evaluate the clinical effect of irbesartan and lercanidipine in treating of senile hypertension. Methods A total of 116 cases of senile hypertension collected from June 2014 to March 2016 were randomly divided into two groups, 59 cases in the treatment group and 57 cases in the control group.The treatment group was treated with 150 mg irbesartan, qd, and 10 mg lercanidipine,qd;while the control group was given 150 mg irbesartan and 12.5 mg hydrochlorothiazide qd. The blood pressure and pulse wave velocity（PWV） of the two groups were compared after 12 weeks treatment. Results After treatment 12 weeks, the 24 h average SBP and DBP, daytime average SBP and DBP, and nighttime average SBP and DBP in the two groups were significantly lower than those of before treatment（P〈0.05）. The SBP and DBP in the treatment group decreased significantly than those in the control group（P〈0.05）. The total con trol rate of blood pressure in the treatment group was significantly higher （P〈0.05）. The brachial ankle pulse wave ve locity（baPWV） and cardio ankle pulse wave velocity （haPWV） of two groups declined significantly than those before treatment（P〈0.05）. Compared the control group, the treatment group was significant lower. No serious untoward reactions appeared in the two groups, the control group had a small number of patients with blood lipid, uric acid and electrolyte. Conclusion Irbesartan and lercanidipine can effectively reduce senile hypertension, with stable and durable antihypertensive effect, good for elderly patients with mild to moderate senile hypertension.
