弹性髓内钉治疗儿童四肢长骨干骨折临床疗效分析

摘　　要：目的 观察钛制弹性髓内钉（titanium elastic nail,TEN）内固定治疗儿童四肢长骨干骨折的临床疗效。方法选择2014年5月-2015年5月我科使用弹性钉治疗儿童四肢骨折45例,回顾性分析患者的手术时间、术中出血量、术后伤口愈合情况及并发症情况。结果 使用Flynn评分标准评估45例患者治疗效果,所有患者均获得骨性愈合,Flynn评分优良率97.8%。所有病例均在术后3-6个月取出髓内钉。术中出现骨质劈裂1例;术后出现弹性钉激惹1例,取出困难5例,缩短畸形1例。全部病例术后无感染、骨不连、骨骺损伤。结论 TEN内固定治疗儿童四肢长骨干骨折有较好的临床疗效,具有创伤小、手术时间短、患者康复快等优点,且并发症较少,是一种安全有效的治疗方法,值得临床推广应用。Objective To observe the clinical efficacy of titanium elastic nail （TEN） internal fixation in the treatment of long bone fractures of limbs in children. Methods 45 children patients with limb fractures were treated with elastic nails from May 2014 to May 2015. The operation time, intraoperative blood loss, postoperative wound healing and complications were retrospectively analyzed. Results The therapeutic effect was evaluated in 45 patients using the Flynn scoring system. Osteal healing was achieved in all patients and the Flynn score excellent and good rate was 97.8%. Intramedullary nail was removed 3-6 months after the surgery in all patients. 1 case of osteolytic cleft was seen in operation; 1 case of elastic nail provocation, 5 cases of removal difficulties, and 1 case of shortened deformity were seen in operation. No infection, nonunion or epiphyseal injury were found in all cases. Conclusion TEN internal fixation is a safe and effective treatment for long bone fractures of limbs in children, which has better clinical curative effects, with advantages of less trauma, shorter operative time, quicker recovery and less complications. It is worthy of clinical promotion and application.
