摘  要:目的分析体外反搏(ECP)对老年冠状动脉粥样硬化性心脏病患者血尿酸(SUA)及醛固酮(ALD)水平的影响,探讨ECP治疗冠状动脉粥样硬化性心脏病的机制。方法选取96例年龄>65岁冠状动脉粥样硬化性心脏病患者,随机分为对照组(32例)、短程治疗组(32例)、长程治疗组(32例)。对照组采用常规治疗,短程治疗组采用常规治疗+ECP治疗15 d,长程治疗组采用常规治疗+ECP治疗35 d。35 d后评价各组治疗效果,酶法检测血尿酸水平,放射免疫法检测血醛固酮水平。结果对照组、短程治疗组、长程治疗组有效率分别为53.1%、81.2%、90.6%;与对照组比较,短程及长程治疗组有效率显著提高(P<0.01);长程治疗组有效率较短程治疗组更高(P<0.05);短程及长程治疗组患者治疗后SUA、ALD水平均下降(P<0.05);长程治疗组SUA、ALD水平下降更明显(P<0.01)。结论 ECP能改善老年冠状动脉粥样硬化性心脏病患者症状,同时能明显降低患者血尿酸及醛固酮水平。Objective To analyze the effects of external counter pulsation on serum uric acid (SUA) and aldosterone (All)) in the elderly pa- tients with coronary atherosclerotic heart disease, and to investigate the new mechanism of coronary atherosclerotic heart disease. Methods A to- tal of 96 elderly patients with coronary atherosclerotic heart disease were recnfited into the study, and randomly assign into 3 groups : control group (n = 32), short-term treatment group (n = 32), and long-term treatment group (n = 32). Three groups were given standard therapies, the short- term treatment group received external counter pulsation with 15 day, and the long-term treatment group received external counter pulsation with 35 day. The clinical efficacies were evaluated after the treatment. Radioimmunoassay was applied to measure the level of ALD from all the patient before and after treatment. An enzymatic method was applied to measure the level of SUA from all the patient before and after treatment. Results The effective rates of three groups were 53.1%, 81.2%, and 90.6%, respectively. The effective rates of the short-term treatment and long-term treat- ment group were obviously higher than the control group (P 〈 0.01 ), and the long-term treatment group was higher than the short-term treatment group (P 〈 0.05). There were significant difference in the level of SUA and ALl) in the short-term treatment and long-term treatment group before and after treament (P 〈 0.05). The level of SUA and ALD were significantly decreased (P 〈 0.01 ) in the long-term treatment group as compared with before and after treament. Conclusion External counter pulsation has obvious therapeutic etticacy in elderly patients with coronary athero- sclerotic heart disease, which can significantly reduce the level of SUA and ALD.
