摘 要:目的探讨经直肠声辐射力脉冲成像(ARFI)对前列腺良恶性病变的诊断临价值。方法经直肠常规超声探及前列腺病变患者71例,依据穿刺活检的病理结果分为恶性病变组和良性病变组;另前列腺无疾病者33例作为对照组。应用ARFI测量其剪切波速度(SWV)。结果前列腺恶性病变组SWV为(3.07±0.31)m/s,良性病变组SWV为(2.65±0.52)m/s,对照组SWV为(2.31±0.61)m/s,组间比较差异均有统计学意义(均P<0.05)。以SWV>2.94 m/s作为前列腺恶性占位的诊断标准,ARFI诊断前列腺良恶性占位的敏感性为85.7%,特异性为65.5%,阳性预测值为78.2%,阴性预测值为76.0%。结论 ARFI技术可以作为前列腺病变早期诊断的一种重要手段。Objective To investigate the diagnostic value of transrectal acoustic radiation force impulse(ARFI)technology in prostatic benign and malignant lesions. Methods Seventy-one patients with prostate lesions were examined by transrectal ultrasound,then these patients were divided into benign and malignant lesion groups according to the results of the biopsy pathology. Another 33 cases without prostatic disease were selected as control group. ARFI were performed to measure the shear wave velocity(SWV)in the different groups. Results The SWV of prostatic malignant lesion group, benign group and control group were(3.07± 0.31)m/s,(2.65 ±0.52)m/s and(2.31±0.61)m/s, respectively, there was difference(all P0.05). The sensitivity and specificity were 85.7% and 65.5%, respectively, the positive predictive value and the negative predictive value were 78.2% and76.0% in the diagnosis of prostate benign and malignant lesions with SWV2.94 m/s as the diagnostic criteria for malignant lesion of the prostate. Conclusion ARFI can be used as an important diagnostic method for early diagnosis of prostatic lesions.
