摘  要:目的探讨组织追踪成像(TTI)测量收缩期二尖瓣环位移(MADs)及二维应变测量左心长轴纵向应变评价左心收缩功能的临床价值。方法心肌梗死患者30例,体检健康者30例为对照组,采用双平面Simpson’s法测量左室射血分数(LVEF),TTI测量室间隔、左室侧壁、左室前壁及左室下壁4个位点的MADs。应用二维应变测量左室长轴纵向应变,分析LVEF与MADs、MADs与左室长轴纵向应变的相关性。结果与对照组比较,心肌梗死组4个位点的MADs均低于对照组(P<0.01),平均MADs与LVEF呈正相关(r=0.85),与左室长轴纵向整体应变呈负相关(r=-0.788)。结论 TTI测量MADs可以简捷、准确地评价心肌梗死的左心收缩功能,且与二维应变所测量左室长轴整体纵向应变相关性良好。Objective To investigate the clinical value of measuring left ventricular systolic function by tissue tracking imaging(TTI) to measure the systolic mitral annulus displacement(MADs) and two-dimensional strain measurement of left ventricular long axis longitudinal strain. Methods Thirty patients with myocardial infarction were enrolled,and thirty healthy subjects served as control group. The left ventricular ejection fraction(LVEF) were measured with routine two-dimensional echocardiography and Simpson double- plane. The maximal displacements(MADs) of four points(ventricular septum, left ventricular lateral wall, left ventricular anterior wall and left ventricular inferior wall)of mitral annulus were measured from apical4- chamber view,3- chamber view and 2- chamber view using tissue tracking imaging(TTI). The longitudinal strain of left ventricle was measured by two-dimensional strain, and the correlation between LVEF and MADs, MADs and longitudinal strain(LS)of left ventricle was analyzed. Results The MADs of four points in all patients with myocardial infarction were significantly lower than those of control group, there was significant difference(P0.01). There was a positive correlation between TMAD and LEVF(r=0.85,P0.01),and a negative correlation between TMAD and LS(r=-0.788,P0.01). Conclusion TTI measurement of MADs can be a simple and accurate method to evaluate the left ventricular systolic function of myocardial infarction, and has a good correlation with the longitudinal strain of the left ventricle.
