吉诺通胶囊联合顺尔宁对分泌性中耳炎的临床疗效及对相关炎性因子的影响

摘　　要：目的:观察吉诺通胶囊联合顺尔宁治疗分泌性中耳炎(SOM)的临床疗效及对患者炎性因子的影响。方法:64例分泌性中耳炎患者随机分为对照组和治疗组,每组32例。对照组SOM患者给予头孢克洛治疗,0.75g/次,每天2次,连用7天;顺尔宁10 mg/次,每天1次,连续服用2周。治疗组SOM患者在此基础上加用吉诺通胶囊,300 mg/次,每日3次。两组疗程均为2周。比较2组患者的临床疗效、鼓室压、语频气导听阈变化、血清和中耳积液中炎症因子变化水平及不良反应。结果:治疗组SOM患者治疗总有效率为87.18%,显著高于对照组的71.05%(P<0.05)。治疗组鼓室压、语频气导听阈值均显著高于治疗前和对照组(P<0.05)。治疗组SOM患者血清和中耳积液中的PAF、IL-6表达水平显著低于对照组(P<0.05),但ET-1表达水平差异无统计学意义(P>0.05);治疗过程中未发现明显不良反应发生。结论:中西医结合治疗对分泌性中耳炎临床效果显著,通过减轻炎症反应促进患者听力恢复。Objective ： To evaluate the clinical efficacy of Jinuotong Capsule and montelukast in the treatment of secre- tory otitis media（SOM） and its influence on inflammatory factor. Methods： Sixty -four patients with SOM were randomly divided into control group and treatment group, 32 cases in each group. Control group was treated with cefaclor orally, 0. 75 g one time,ql2h for 7 d. Patients in treatment group were treated with Jinuotong Capsule additionally, 2 weeks a course. The clinical efficacy, tympanic pressure, speech frequency air conduction threshold, levels of inflammatory factor in serum and middle ear fluid（MEF） and adverse reactions were compared between the two groups. Results ： The effective rate in treatment group was 87.18 %, significantly higher than 71.05 % in control group （P 〈 O. 05 ）. After treatment, the levels of tympanic pressure and speech frequency air conduction threshold in treatment group were significantly higher than those in control group（P 〈0. 05）. The levels of PAF and IL-6 in serum and MEF in treatment group were signifi- cantly lower than those in control group （ P 〈 0. 05 ）, but the level of ET - 1 had no significant difference （ P 〉 0.05 ）. During treatment, it showed no significant adverse reactions. Conclusion： The integrated traditional Chinese and western medicine has obvious curative effect on SOM syndrome, promoting the recovery of patients＇hearing by reducing the inflam-matory reaction.
