清热生肌中药熏洗辅助常规创面消毒治疗手外伤术后感染临床研究

摘　　要：目的：探讨清热生肌中药熏洗辅助常规创面消毒治疗手外伤术后感染临床效果。方法：研究对象选取我院2012年1月~2015年5月收治疗手外伤术后伤口感染患者共140例,其中78例行常规创面消毒治疗设为对照组,62例则在此基础上加用清热生肌中药熏洗治疗设为观察组;比较两组患者临床疗效、创面愈合时间、随访创面细菌培养阳性率及治疗前后肉芽组织和分泌物评分等。结果：观察组患者临床疗效显著优于对照组,差异具有显著性意义（P〈0.05）;观察组患者创面愈合时间显著短于对照组,差异具有显著性意义（P〈0.05）;观察组患者随访细菌培养阳性率均显著低于对照组,差异具有显著性意义（P〈0.05）;观察组患者治疗后肉芽组织和分泌物评分均显著低于对照组、治疗前,差异有统计学意义（P〈0.05）。结论：清热生肌中药熏洗辅助常规创面消毒治疗手外伤术后感染可有效加快创面愈合进程,提高细菌清除效果,具有临床应用价值。Objective： To investigate the clinical effects of using Qingre Shengji Chinese herbs for fumigation and washing assisted with conventional wound disinfection in the treatment of infection after hand injury operation. Methods： 140 patients with infection after hand injury operation were chosen in the period from January 2012 to May 2015 in our hospital and divided into both group including controlled group（ 78patients） with conventional wound disinfection and observation group（ 62patients） with Qingre Shengji Chinese herbs for fumigation and washing on the basis of the controlled group; and the clinical effects,wound healing time,culture positive rate of bacterial in the wound with follow-up,the scores of granulation tissue and secretion before and aftertreatment of both groups were compared. Results： The clinical effects of the observation group was significantly better than the controlled group（ P〈0. 05）. The wound healing time of the observation group was significantly shorter than the controlled group（ P〈0. 05）. The culture positive rate of bacterial in the wound with follow-up of the observation group was significantly lower than the controlled group（ P〈0. 05）. The scores of granulation tissue and secretion after treatment of the observation group was significantly lower than the controlled group（ P〈0. 05）. Conclusion： Using Qingre Shengji Chinese herbs for fumigation and washing assisted with conventional wound disinfection in the treatment of severe wound infection after hand injury operation can efficiently promote wound healing process and improve the effect of bacterial clearance,and has clinical application value.
