摘 要:目的分析兔VX2肝肿瘤模型的全肝CT灌注成像灌注参数与不成对动脉(UAs)的相关性,为探讨肿瘤血管生成状况及与灌注成像表现的本质联系提供依据。方法采用健康清洁级新西兰大白兔55只,其中假手术对照组5只,剔除经解剖和病理证实为未成功手术成瘤的,最终实验组动物为42只。术后2周获得兔VX2肝肿瘤边缘、瘤周肝及假手术组的肝脏灌注血流量(BF)、肝脏灌注血容量(BV)、肝动脉灌注量(ALP)、门静脉灌注量(PVP)、肝动脉灌注指数(HPI)等参数值,并于检查后通过免疫组织化学染色获得肿瘤标本的UAs值。结果实验组VX2瘤边缘BF、BV、ALP、PVP、HPI与实验组瘤周肝比较,差异均有统计学意义(P<0.05);实验组瘤周肝各灌注成像参数值与假手术对照组比较,差异无统计学意义(P>0.05)。BF、BV、ALP、HPI与UAs呈正相关(r=0.475、0.434、0.635、0.323,P<0.05或0.01),PVP与UAs呈负相关(r=-0.365,P<0.05)。其中以ALP与UAs的相关系数最高(r=0.635,P<0.01)。结论全肝CT灌注可定量评估兔肝VX2瘤的肝动脉和门静脉灌注信息,并可在一定程度上反映肿瘤血管生成状况。Objective To analyze the correlation between the parameters of total liver CT perfusion imaging in rabbit VX2 liver tumor model and unpaired arteries(UAs) to provide a basis for the intrinsic contact between the tumor angiogenesis status and perfusion imaging manifestations. Methods Fifty-five healthy clean class New Zealand while rabbits were adopted. Among them,5 cases were in the sham operation control group,those were removed due to un-succeeded operative tumor formation verified by anatomy and pathology,and 42 cases were finally the experimental animals. The parameters of blood flow(BF),blood volume(BV),arterial liver perfusion(ALP),portal liver perfusion(PVP)and hepatic perfusion index(HPI) of the tumor rim,surrounding liver tissue and sham-operated group were obtained at postoperative 2 weeks. After the examination,the UAs values of tumor specimens were obtained by immunohistochemical staining. Results BF,BV,ALP,PVP and HPI had statistically significant differences between the tumor rim and the paracancerous liver tissue(P〈0.05),but which had no statistically significant difference between the paracancerous liver tissues in the experimental group and the sham operation group(P〈0.05). BF,BV,ALP and HPI had positive correlation with UAs(r=0.475,0.434,0.635,0.323,P〈0.05 or 0.01),PVP had negative correlation with UAs(r=-0.365,P〈0.05). Among them,ALP had the highest correlation coefficient with UAs(r=0.635,P〈0.01). Conclusion Total liver CT perfusion can quantitatively evaluate the liver artery and portal vein perfusion information of the rabbit liver VX2 tumor,and can reflect the status of tumor angiogenesis to some extent.
