110例先天性晶状体半脱位病人的手术室护理

摘　　要：[目的]探讨张力环植入和人工晶状体缝合两种固定方式治疗先天性晶状体半脱位的手术疗效,同时总结手术室护理体会。[方法]将110例先天性晶状体半脱位纳入研究范围,将所有病人按照随机数字表法分为对照组和观察组各55例,分别接受张力环植入术和人工晶状体缝合术。术后随访至少2年,观察术后视力恢复、人工晶状体脱落情况、低眼压发生率及其他手术相关并发症。在此基础上总结护理操作形成手术室护理流程。[结果]在视力恢复方面,两组病人恢复情况无区别(P>0.05)。术后眼部不适方面,术后1个月时对照组病人发生眼部不适感较观察组明显较高(P<0.05)。3个月后组间疼痛不适感无明显区别(P>0.05),对照组低眼压发生率明显较观察组低,两者差异具有统计学意义(P<0.05)。在人工晶状体脱落率方面,观察组发生脱落的几率较张力环植入组明显较低(P<0.05)。在切口渗液方面,缝合组发生几率较对照组明显较高(P<0.05)。[结论]两种手术方式均可有效改善病人视力,在术后人工晶状体再脱位和低眼压方面,两种手术方式各有优劣,需根据病人多方面因素综合评估。同时规范手术室护理操作,提高医护配合熟练度能有效降低手术风险。Objective：To probe into the surgical effect of two fixed models of tension ring implantation and intraocular lens suture for patients with congenital lens subluxation,and summarize the nursing experience of operating room.Methods：A total of 110 cases of patients with congenital lens subluxation from February 2012 to May 2015 were included into the study.All patients were randomly divided into control group and observation group,55 cases in each.The patients in control group were treated with tension ring implantation and the patients in observation group received intraocular lens suture.At least 2years of follow-up after surgery,postoperative visual acuity,intraocular lens shedding,low intraocular pressure and other complications were observed.On this basis,the nursing operation was summarized and formed the operation room nursing process.Results：There was no statistically significant difference in the recovery of visual acuity between the two groups,the difference was not statistically significant（P〉0.05）.At one month after the operation,the ocular discomfort of patients in control group was significantly higher than that in observation group,and the difference was statistically significant（P〈0.05）.After three months,there was no statistically significant difference in pain between the two groups,the difference was not statistically significant（P〉0.05）,the incidence of low intraocular pressure in control group was significantly lower than that in observation group,the difference was statistically significant（P〈0.05）.The incidence of shedding of artificial lens in observation group was significantly lower than that in control group（P〈0.05）.The incidence of incision leakage in observation group was significantly higher than that in control group,and the difference was statistically significant（P〈0.05）.Conclusion：Both surgical methods could effectively improve the patients＇ visual acuity,in posterior dislocation and low intraocular pressure,two kinds of surgical methods
