摘  要:背景:高分子载体材料是药物缓释及控释制剂的重要组成部分,也是影响药效的主要因素,了解适合不同药物的药物载体材料,有助于达到理想的药物控制释放效果。目的:阐述药用高分子载体材料的特点,分析缓释及控释制剂在高血压治疗中应用。方法:(1)检索PubM ed数据库和万方数据库,中文检索词为"高分子;缓释材料;控释材料;壳聚糖;环糊精;丝素蛋白;聚酸酐;聚乳酸"英文检索词为"Polymers;sustained-release material;controlled-release material;chitosan;cyclodextrin;Silk fibroin;Poly acid anhydride;Polylactic acid",纳入文献时间限定为2006至2015年。按纳入排除标准选择30篇文献进行分析;(2)通过检索万方数据库分析国内2010至2015年缓释及控释制剂在高血压治疗中的应用趋势。结果与结论:(1)作为药物释放载体的高分子材料,应当具备无毒或低毒,良好的生物相容性和生物降解性等基本条件,可以分为天然高分子载体材料(如多糖类、蛋白质类材料)和合成高分子载体材料(如聚乳酸、聚酸酐、多肽与氨基酸类聚合物等材料),近年出现高分子复合材料能够克服单一材料的缺点,拓宽了缓释载体的范围,有很大的研究空间。尽管目前针对缓、控释药物的研制已取得了可喜的成果,但还未能完全达到高效、速控甚至智能化的要求;(2)万方数据库收录缓释及控释片治疗高血压的文献数量从2010至2015年逐年上升,说明临床上高血压治疗中缓释及控释片的应用逐年普及,报道较多的有硝苯地平缓释或控释片、非洛地平缓释片和美托洛尔缓释片。BACKGROUND: Polymeric material, an important carrier used in the sustained-release and controlled-release system, is a major factor influencing drug efficacy; so understanding of different carrier materials contributes to obtain an ideal control-released effect. OBJECTIVE: To clarify the characters of medical polymeric materials, and analyze their application in the sustained-release and controlled-release system in the treatment of hypertension. METHODS: An online research was performed in databases of PubMed and WanFang for relative literatures published from 2006 to 2015 using the keywords of "polymers; sustained-release material; controlled-release material; chitosan; cyclodextrin; ilk fibroin; poly acid anhydride; polylactic acid" in Chinese and English, respectively. Totally 30 literatures were enrolled for analysis based on the inclusion and exclusion criteria. Analyzing the application tendency of sustained-release and controlled-release system in the treatment of hypertension was conducted by retrieving WanFang database between 2010 and 2015 in China. RESULTS AND CONCLUSION: Polymeric materials used for release carriers should be non-toxic or low toxic and hold good biocompatibility and biodegradability, including natural polymeric materials (polysaccharides and proteins) and synthetic polymeric materials (polylactic acids, polyanhydrides, polypeptides and amino acids). These new polymeric materials overcome some shortcomings of the single material and expand the range of controlled-release carriers showing a light application prospect. Studies on the sustained-release and controlled-release system have achieved satisfactory outcomes, but questions of efficiency, speed-control and intelligentization remain to be resolved.
