摘  要:目的评价肩峰成形在关节镜下全层肩袖撕裂修复术中的作用。方法选取因肩袖损伤在我院接受手术治疗的患者共76例,随机分为肩峰成型组和非肩峰成型组。肩峰成型组40例;非肩峰成型组36例。术前术后采用加州大学(UCLA)肩关节评分系统、美国肩肘外科医师(ASES)以及西安大略肩袖疾病评分(WORC)评价肩关节功能。两组计量数据采用独立样本T检验进行统计学分析。结果 ACR-A组的手术时间为70.3 4.1 min,ACR组的手术时间为59.5 7.3 min,两组比较差异有统计学差异(P<0.05),而术后3月、6月、12月的WORC评分、UCLA评分及ASES评分的差异均无明显统计学意(P>0.05)。结论本研究未发现肩袖全层撕裂修复过程中,ACR-A组和ACR组术后的肩关节功能有显著差异,但并不就此否定肩峰成形术在肩袖损伤修复过程中的重要作用,未来需分析不同肩峰形态对肩袖修复术临床效果的影响,并进行长期随访。Objective To report the clinical effect of acromioplasty on full-thickness rotator cuff tears repair under arthroscope.Methods Patients undergoing full-thickness rotator cuff tears were randomized into acromioplasty and nonacromioplasty groups.Acromioplasty group was 40 cases while nonacromioplasty group was 36 cases. The primary results were measured by Western Ontario Rotator Cuff(WORC) index, American Shoulder and Elbow Surgeons(ASES) shoulder assessment system and assessment system of University of California at Los Angeles(UCLA). Results were measured preoperatively and at three, six, twelve months after surgery. Results There were significant differences in operation time between two groups(P〈0.05)while there were no differences in UCLA,WORC or ASES scores between two groups at any time point(P〉0.05). Conclusion This investigation do not demonstrate any differences in clinical outcomes after arthroscopic repair of full-thickness rotator cuff tears with or without acromioplasty during short-term follow-up.However, specific analysis of acromion type and long-term follow-up are needed to determine whether functional or structural differences exist in patients undergoing arthroscopic rotator cuff repair with or without acromioplasty.
