全关节镜下单排锚钉修复肩袖损伤的疗效分析

摘　　要：目的探讨全关节镜下单排锚钉修复肩袖损伤的手术疗效。方法分析2013年3月至2016年3月在我院确诊为肩袖损伤的45例患者资料。根据Coifed分型标准：部分肩袖损伤8例,中小型撕裂21例,大型撕裂12例,巨大型撕裂4例。所有患者均行全关节镜下单排锚钉修复术。术后根据肩关节VAS评分和UCLA评分判定手术疗效。结果本组病例随访10个月~18个月,平均13.7个月。肩关节VAS评分由术前（7.3±1.2）分降低至术后（1.3±0.7）分,差异具有统计学意义（P〈0.05）;UCLA评分由术前（19.7±3.2）分提高至术后（31.9±2.7）分,差异具有统计学意义（P〈0.05）。术后41例无疼痛症状,4例偶有不适或轻度疼痛;43例可完全恢复伤前运动水平,2例可从事肩关节以上水平的工作。结论全关节镜下单排锚钉修复术具有创伤小,术野清晰,并发症少等优点,且临床疗效满意。Objective To investigate the effect of all-arthroscopic single-row suture on rotator cuff repair. Methods 43 cases with rotator cuff injury being treated in our hospital from March 2013 to March 2016 were collected. All cases were divided into four groups according to Coifed classification. Among them, 8 cases belonged to partial rotator cuff injury, 21 cases were moderate rotator cuff injury, 12 cases suffered from severe injury and 4 cases were most severe injury. All cases were received single-row suture therapy under all-arthroscopy. Theraputic effect was evaluated based on VAS score and UCLA score. Results All cases had been followed up for 10 to18 months（average 13.7 months）. After surgery, the VAS score decreased to 1.3±0.7 from 7.3±1.2 before operation（P〈0.05）; UCLA score increased to 31.9±2.7 from 19.7±3.2（P〈0.05）. At last, 41 cases didn＇t appear pain symptoms, 4 cases felt a little discomfort or mild pain, 43 cases recovered thoroughly, 2 cases could do some works beyond the shoulder joint. Conclusion It have many advantages for single-row suture under all-arthroscopy to cure rotator cuff injury, they include small trauma, clear operation field, less complications,and satisfactory effectiveness.
