摘 要:目的研究并对比腰椎间盘退行性病变的MR1表现和病理学改变。方法选择15具新鲜尸体标本作为研究对象,总共有33个腰椎间盘,对这15具标本进行MRI检查,并对33个腰椎间盘标本进行病理大切片,切片层面共89层,对病理诊断表现和MRI表现进行对比分析。结果 33个腰椎间盘标本均出现纤维环退行性病变,有21个腰椎间盘标本在T2W1显像中呈带状高信号;病理可见有21个腰椎间盘髓核内有坏死的细胞碎渣残留,T2W1加权像中显示为斑点状低信号,病理可见23个内折纤维环和19个外折纤维环,内折纤维环表现为内突向折曲低信号,外折纤维环表现为折角低信号;病理可见有2个椎间盘中的纤维环内有大裂隙,T2Wl加权像中有低信号;病理可见有7个腰椎间盘发生退行性病变,其纤维组织生长快速且呈分支状,T2W1加权像中显示为低信号。结论对腰椎间盘退行性病变进行MRI检查,能够有效提高其确诊率,为腰椎间盘退行性病变的治疗提供参考依据。Objective To study and contrast of the degeneration of lumbar intervertebral disc MR1 manifestation and pathological changes. Methods 15 fresh cadaver specimens as the research object,a total of 33 lumbar intervertebral disc, MRI was performed on the15 specimens, and pathological sections of 33 lumbar intervertebral disc specimens, slice level a total of 89 layers, the comparative analysis of the pathological diagnosis of manifestation and MRI. Results The 33 lumbar disc specimens were appeared annulus degeneration, 21 lumbar intervertebral disc specimens with zonal high signal in T2W1 imaging; pathology visible 21 lumbar intervertebral disc nucleus pulposus cells with necrosis of the slag residue, T2W1 weighted images showed patchy low signal pathology may see 23 inflexed fiber the outer ring and 19 fold fiber ring, inflexed fiber ring is apodeme to flex low signal, deflexed fiber ring angle showed low signal; pathological 2intervertebral disc annulus in large fissures, T2 Wl weighted images with low signal; Pathology There were 7 lumbar intervertebral disc degeneration occurs, the rapid growth of fibrous tissue and a branched, T2W1 weighted images showed low signal. Conclusion MRI examination of lumbar intervertebral disc degeneration, can effectively improve the accuracy of diagnosis, and provide the reference for the treatment of lumbar disc degeneration.
