摘 要:目的探讨骨转移癌患者跟骨骨密度(BMD)与血清骨钙素(BGP)、骨性碱性磷酸酶(B-ALP)、I型前胶原氨基端前肽(PINP)水平的相关性。方法采用定量超声法、化学发光免疫分析法(CLIA法)、酶联免疫法(ELISA法)测定101例骨转移癌患者跟骨BMD以及血清BGP、B-ALP、PINP水平,并以101例健康人群作对照组,然后按性别分组,分别进行性别内的患病组与对照组比较。结果骨转移癌患者跟骨BMD T值、Z值下降(P<0.05),与年龄呈负相关、与体质量呈正相关(P<0.05),女性患者Z值还与身高、BMI正相关(P<0.05);血清BGP、B-ALP、PINP水平升高(P<0.05),与BMD T值、Z值负相关(P<0.05)(女性患病组Z值与BGP无相关关系)。结论骨转移癌患者跟骨BMD下降,骨形成标志物BGP、B-ALP、PINP升高,两者存在一定相关性;跟骨BMD与骨形成标志物联合检测,可较好地评估患者局部及整体骨量变化。Objective To investigate the level of calcaneat bone mineral density (BMD) and three kinds of bone formation markers, including osteocalcin (BGP), bone alkaline phophatase (B -ALP), and aminoterminal propeptide of type I procollagen (PINP), patients with bone metastasis; and to discuss their relevance. Methods... The quantitative ultrasound method, chemiluminesent lmmunoassay assay (CLIA) and enzyme linked immunosorbent assay (ELISA) were used to determine T value and Z value of the calcaneal BMD, and BGP, B - ALP, PINP in serum of 101 cases with bone metastasis. The correlations were analyzed. Results The BMD T and Z values of patients with bone metastasis were significantly reduced (P 〈 0. 05 ), and negatively correlated to age, and positively correlated' to body mass (P 〈 0. 05). The Z value was significantly negatively correlated to height and BMI in the female patients (P 〈 0. 05 ). The levels of BGP, B - ALP and PINP in serum of disease group were significantly increased (P 〈 0. 05 ) ; and significantly negatively correlated to BMD T and Z values (P 〈0. 05, except for Z value and BGP in female). Conclusion The calcaneal BMD of patients with bone metastasis decreases, with increase in the levels of bone formation markers as BGP, B - ALP, and PINP. The union detection can be used for accurate assessment the changes of the local and general of BMD.
