摘  要:目的探讨应用锥形束CT(cone-beam CT,CBCT)观察新疆维吾尔族成人双侧上颌第二前磨牙的牙根和根管系统的解剖形态中的价值。方法新疆维吾尔族成人患者100例,依据CBCT图像分析双侧上颌第二前磨牙的牙根数目、根管数目、形态,两根管口间距离,多根牙根分叉和多根管分叉位置,依据Vertucci分类对根管进行分型。结果200颗上颌第二前磨牙中单根176颗(88.00%),双根22颗(11.00%),三根2颗(1.00%);Ⅰ型根管105颗(52.50%),Ⅱ型根管46颗(23.00%),Ⅲ型根管16颗(8.00%),Ⅳ型根管23颗(11.50%),Ⅴ型根管5颗(2.50%),Ⅵ型根管2颗(1.00%),Ⅶ型根管1颗(0.50%),Ⅷ型根管2颗(1.00%);176颗单根牙中Ⅰ型根管105颗(59.66%),22颗双根牙中Ⅳ型根管18颗(81.82%),2颗三根牙根管均为Ⅷ型;双根管的上颌第二前磨牙两根管口间距离为1.02~3.82mm;多根牙根分叉在根冠、根中、根尖1/3处均有分布,根管分叉位置多在根冠1/3偏向根冠及根中1/3交界处。结论新疆维吾尔族成人上颌第二前磨牙根管系统解剖形态复杂,多根管比例高,CBCT可为根管治疗提供准确信息。Objective To investigate the cone-beam CT (CBCT) findings of root canal morphology of bilateral maxillary second premolar in Uygur adults in Xinjiang. Methods A total of 100 patients received CBCT to analyze the root number, root canal number and root canal morphology of maxillary second premolar, the distance between two root canal orifices, and multi-root and multicanal bifurcation positions. Vertucci's method was adopted to classify the root canal types. Results In 200 teeth from 100 patients, there were 176 single-root teeth (88.00%) , 22 hi-root teeth (11.00%), and 2 tri-rootteeth (1.00%). There were 105, 46, 16, 23, 5, 2, 1 and2 teeth of type Ⅰ , Ⅱ , Ⅲ, Ⅳ, Ⅴ, Ⅵ, Ⅶ, and root canals respectively, accounting for 52.5%, 23.00%, 8.00%, 11.50%, 2. 50%, 1.00%, 0.50%, and 1.00%. In 176 single-root teeth, there were 1.05 teeth of type Ⅰ root canal (59. 66%), in 22 bi-root teeth, there were 18 teeth of type IV root canal (81.82%), and both 2 tri-root teeth were type Ⅷ root canal. The distance between two root canal orifices of maxillary second premolars with bi-root canal was approximately 1.02 to 3.82 mm. The bifurcation position of multi-root teeth distributed in the crown, middle and tip part of root. The bifurcation position of maxillary second premolar root canals focused on the one third of the root. Conclusion The canal anatomy of maxillary second premolar is complex, the percentage of multi-root canals is high, and CBCT can provide an accurate basis for root canal treatment in Uygur patients in Xinjiang.
