摘 要:目的比较使用3D打印个性化截骨工具(patient-specific instrumentation,PSI)与传统截骨方式行膝关节置换(total knee arthroplasty,TKA)的短期临床疗效。方法 46例行TKA患者按随机数字表法分为两组各23例,PSI组应用3D打印PSI,传统组应用传统截骨工具。比较两组的手术时间、术后24小时引流量、术后输血率、术后冠状位下肢力线离群值即股骨-胫骨力线轴(MFT)夹角>3°比例、术后6月美国膝关节协会评分(Knee Society Score,KSS)。结果 PSI组手术时间短于传统组,术后24小时引流量少于传统组,差异有统计学意义(P<0.05)。两组术后输血率、术后冠状位下肢力线离群值、术后6月KSS评分比较差异无统计学意义(P>0.05)。结论应用3D打印PSI行TKA手术与应用传统截骨工具行TKA手术相比,手术时间更短,术后引流量更少,但输血率并没有明显降低,且在下肢力线及术后早期膝关节功能康复方面并没有明显优势。Objective The purpose of this study was to evaluate the short-term clinical effect of total knee arthroplasty ( TKA) between 3D-printing patient-specific instrumentation (PSI) and conventional instrumentation.Methods Forty-six patients undergone TKA were randomly divided in to A and B groups,23 in each group.The group A used 3D-printing PSI while the group B used conven-tional instrumentation.The operation time,24 h postoperative drainage volume,blood transfusion rates,ratio of angle between femoral and tibial ( AFT) 〉3°and American Knee Society Score( KSS) after 6 months of surgery were compared between the two groups.Re-sults The average surgical time was shorter and the average 24 h postoperative volume of drainage was less in the group A than those in the group B (P〈0.05).There were no significant differences in blood transfusion rates,the percentage of outliers for alignment and the KSS after 6 months of surgery (P〉0.05).Conclusion The 3D-printing PSI in TKA can shorten surgical time and reduce postop-erative volume of drainage when compared to conventional instrumentation.However,blood transfusion rates are not reduced,and there is no significant advantages in low limb force line and early recovery of postoperative knee function.
