县级医院开展CT引导下经皮肺穿刺活检术诊断肺周围性肿块的实用价值探讨

摘　　要：目的探讨县级医院开展CT引导下经皮肺穿刺活检术诊断肺周围性肿块的实用价值。方法 2010年1月至2016年1月我院收治的肺部占位性病变患者183例,以开展经皮肺穿刺活检术时间为节点分为A、B两组,分别计算两组诊断结果阳性率、转诊率,同时分析并总结在CT引导下经皮肺穿刺活检术的技术方法、并发症情况。结果 A组最终明确诊断56例(56/94),阳性率59.6%,B组最终明确诊断80例(80/89),阳性率89.9%,两组患者诊断阳性率比较差异有统计学意义(P<0.001)。结论 CT引导下经皮肺穿刺活检术诊断肺周围性肿块阳性率高,并发症少,操作容易掌握,县级医院开展具有很好的实用价值。Objective To investigate the application value of CT-guided percutaneous lung biopsy for peripheral lung mass at county-level hospitals.Methods One hundred and eighty-three patients with peripheral lung mass from January 2010 to January 2016 were treated in our hospital.They were divided into group A and group B according to the time node of CT-guided percutaneous lung biopsy.The positive diagnostic rate and referral rate of the two groups were calculated,respectively.The technical methods and compli-cations of CT-guided percutaneous lung biopsy were summarized.Results The final diagnosis of group A was clear in 56 cases （56/94）,and the positive rate was 59.6%.The final diagnosis of B group was clear in 80 cases （80/89）,and the positive rate was 89.9%.The difference was statistically significant between the two groups （P〈0.0001）.Conclusion CT-guided percutaneous lung biopsy in the diagnosis of pulmonary mass is practical at county-level hospitals for its high positive rate, few complications and easy grasp of the operation.
