穴位神经刺激仪对剖宫产术后慢性疼痛的影响探讨

摘　　要：目的 探究韩氏穴位神经刺激仪对剖宫产术后慢性疼痛治疗效果.方法 选择2015年3月至2016年3月本院收治的106例剖宫产术后较慢性疼痛者为研究对象,将其分成两组,对照组行常规治疗方式,以此为基础,观察组行穴位神经刺激仪治疗法,观察术中情况和术后焦虑抑郁分数、术后3个月CPSP情况、48h和72 h的VAS评分以及血清IL-10和IL-6指标.结果 两组患者手术情况与手术后焦虑抑郁评分差异无统计学意义（P＞0.05）;在结束手术后72 h之内,观察组患者的VAS评分显著降低,对照组患者该项指标呈现出增高趋势.48 h时,两组患者的VAS分数差异无统计学意义（P＞0.05）;72 h后,和对照组相比,观察组显著较好,差异有统计学意义（P＜0.05）;手术后,所有患者的IL-6呈现出了显著上升趋势,在完成手术后1d,该值达到顶峰,和术前相比,术后3dUL-6指标显著增高（P＜0.05）.观察组患者3d后,IL-6指标明显比对照组低,差异有统计学意义（P＜0.05）;对照组患者手术后72 h内IL-10组间数据差异无统计学意义（P＞0.05）;观察组患者术后72 h与术前相比显著增高,也明显比对照组高（P＜0.05）;术后3个月,观察组患者出现CPSP概率为7.55％,对照组为18.87％,差异有统计学意义（P＜0.05）.结论 对于剖宫产后疼痛者,使用韩式疗法可取得满意临床效果,值得进一步推广使用.Objective To explore the therapeutic effect of acupoint nerve stimulator in the treatment of chronic pain after cesarean section.Methods 106 patients with chronic pain after cesarean section treated at our hospital from March,2015 to March,2016 were selected as study objects and were divided into a control group and an observation group.The control group were routinely treated;in addition,the observation group were treated by acupoint nerve stimulator.The scores of anxiety and depression during and after operation,CPSP condition 3months after operation,VAS scores 48 and 72 h after operation,and levels of serum IL-10 and IL-6 were observed in both groups.Results There were no statistical differences in the scores of anxiety and depression during and after operation between these two groups （P〉0.05）.The VAS was lower in the observation group and higher in the control group 72 h after than before operation,with a statistical difference between these two groups （P〈0.05）.There was no statistical difference in VAS 48 h after operation between these two groups （P〉0.05）.The IL-6 level increased in both groups after operation and reached the peak 1 d after operation and was obviously higher 3 d after than before operation in both groups （P〈0.05） but were much lower in the observation group than in the control group 3 d after operation （P〈0.05）.The IL-10 level was obviously higher 72 h after than before operation in the observation group and were in the observation group than in the control group 72 h after operation （P〈0.05）.The incidence of CPSP was 7.55% in the observation group and was 18.87% in the control group,with a statistical difference （P〈0.05）.Conclusion Acupoint nerve stimulator in treatment of chronic pain after cesarean section is effective,so it is worth being generalized.
