摘  要:目的研究磁共振(MRI)量化检查在胎儿肺发育不良中的诊断价值。方法选取在我院经MRI检查诊断为胎儿肺发育不良的孕妇32例(观察组);另选取同期超声检查显示胎儿肺发育正常但因为其他系统异常而进行MRI检查的孕妇60例(对照组),以产后随访、尸检等结果为确诊依据,分析MRI对胎儿肺发育不良的诊断效果,并分析胎儿总体积(FBV)、肺体积(TFLV)、肺-肝信号强度比(LLSIR)等指标在不同孕周的变化情况,进一步比较两组上述指标的差异。结果观察组入院首次MRI检查胎儿TFLV和LLSIR均较对照组低(P<0.05);胎儿FBV与TFLV、TFLV与孕周、LLSIR与孕周均呈正相关(P<0.05),且随孕周推移观察组TFLV、LLSIR整体低于对照组。结论 MRI诊断胎儿肺发育不良效果显著,且量化检测TFLV、LLSIR有利于评估胎儿肺发育及病变程度。Objective To study the value of MRI quantitative examination in the diagnosis of fetal lung dysplasia. Methods 32 pregnant women diagnosed by MRI as fetal lung dysplasia in our hospital were selected to constitute an observation group, and another 60 pregnant wom en received MRI due to other system anomalies but with normal fetal lung development by ultrasonic examination at the corresponding period were selected to constitute a control group. The effects of MRI in the diagnosis of fetal lung dysplasia were analyzed based on the results of postpartum follow-up visit and autopsy. The changes of such indexes as FBV, TFLV and LLSIR in different gestational weeks were analyzed. The difference in such indexed was further compared. Results The fetal TFLV and LLSIR examined by MRI upon the admission in the observation group were lower than those in the control group(P〈0.05),and the FBV and TFLV, TFLV and gestational week, LLSIR and gestational week were positively correlated respectively(P〈0.05).With the passage of gestational week, the TFLV and LLSIR were lower than those in the control group. Conclusion MRI has significant effects on the diagnosis of fetal lung development, and quantitative detection of LLSIR and TFLV can help to assess the extent of fetal lung development and disease.
