血清PG水平与HP感染相关性分析

摘　　要：目的研究血清胃蛋白酶原(PG)与幽门螺杆菌(HP)感染的相关性。方法检测986例患者血清PGⅠ、PGⅡ、PGⅠ/PGⅡ比值,采用呼气试验检测患者HP,分析HP阳性与阴性患者血清PG水平的差异,对血清PG水平与HP感染进行相关性分析。结果 HP阳性患者PGⅠ水平(72.6±23.2)μg/L较HP阴性患者(116.2±32.8)μg/L显著降低(P<0.01),而PGⅡ水平显著升高(13.6±8.4)μg/L vs(8.3±3.2)μg/L(P<0.01),PGⅠ/PGⅡ比值显著降低(5.8±3.2)vs(13.2±6.8)(P<0.01)。PGⅠ<67μg/L、PGⅡ>15μg/L及PGⅠ/PGⅡ<7.5者在HP阳性组(42.7%、28.4%、44.6%)较HP阴性组(7.9%、7.5%、12.1%)均显著增多(P<0.01)。相关性分析发现HP阳性率与PGⅠ、PGⅠ/PGⅡ及PGⅡ水平显著相关(r=-0.620、-0.602、0.426,P<0.01)。结论血清PG水平与HP感染相关,PGⅠ及PGⅠ/PGⅡ降低或PGⅡ升高有助于预测HP感染。Objective To study the correlation between serum pepsinogen（PG） and Helicobacter pylori（HP） infection.Methods The serum PG II, PG I, PG I/PG II ratio of 986 patients were detected. Their HP was detected by means of breath test to analyze the difference between the serum PG levels of patients with positive and negative HP and analyze the correlation between serum PG levels and HP infection. Results The PG I levels（72.6±23.2）μg/L of patients with positive HP were significantly lower than those of patients with negative HP（116.2±32.8）μg/L（P〈0.01）, while PGⅡ levels were much higher（13.6± 8.4）μg/L vs（8.3±3.2）μg/L,（P〈0.01） and PG I/PG Ⅱ ratio much lower（5.8±3.2）vs（13.2±6.8）（P〈0.01）. Patients with PG Ⅰ67 μg/L, PG Ⅱ15 μg/L and PGⅠ/PG Ⅱ 7.5 were in positive HP group（42.7%, 28.4%, and 44.6%） and were much higher than those in the negative HP group（7.9%, 7.5%, and 12.1%）（P〈0.01）. According to the correlation analysis, HP positive rate was significantly correlated to the levels of PGⅠ, PG I/PG Ⅱ and PG Ⅱ（r=-0.620,-0.602, and 0.426; P〈0.01）. Conclusion Serum PG levels are correlated to HP infection.The decrease of PGⅠ and PGⅠ/PGⅡ or increase of PGⅡ helps to predict HP infection.
