摘　　要：目的 探讨无创通气联合雾化吸入早期治疗对重症哮喘患者临床症状及血气指标的影响。方法 选择94例重症哮喘患者纳入研究,采用随机数字表法分为观察组和对照组,每组47例,观察组患者接受无创通气联合布地奈德＋沙丁胺醇雾化吸入治疗,对照组患者接受布地奈德＋沙丁胺醇雾化吸入治疗,比较两组患者临床症状、血气指标及治疗效果。结果 观察组总有效率为97.87%,明显高于对照组的87.23%（χ^2=3.859,P〈0.05）;咳嗽、气促、喘息、哮鸣音消失时间均明显短于对照组（t=6.109~7.172,P〈0.05）;血氧饱和度（Sa O2）、血氧分压（Pa O2）明显高于对照组,二氧化碳分压（Pa CO2）明显低于对照组（t=5.749~6.912,P〈0.05）。结论 无创通气联合雾化吸入治疗有助于促进临床症状缓解,改善气道通气功能和肺泡氧合功能,纠正血气指标,提高治疗效果。Objective To study the effect of noninvasive ventilation combined with inhalation early treatment on clinical symptoms and blood gas index of severe patients with asthma. Methods 94 patients with severe asthma in our hospital from September 2012-September 2014 were enrolled for the study and randomly divided into observation group and control group. The observation group patients received noninvasive ventilation＋salbutamol and budesonide inhalation therapy , and control patients received budesonide＋salbutamol inhalation therapy. Then clinical symptoms , blood gas index, therapeutic effect were compared between two groups. Results The effective rate of observation group （97.87%） was significantly higher than that of the control group （87.23%） （χ^2=3.859, P〈0.05）. Cough, shortness of breath, breathing spell, and wheezing disappear time were significantly shorter than those of the control group （t=6.109~7.172, P〈0.05）. SaO2 and PaO2 were significantly higher than those of the control group , while PaCO2 was significantly lower than that of the control group （ t=5.749~6.912, P〈0.05）. Conclusion Noninvasive ventilation combined with inhalation therapy contributed to the remission of clinical symptoms, improvement of airway function, alveolar oxygenation, and correction of blood gas, showing a better therapeutic effect.
