摘  要:目的研究红细胞分布宽度与冠状动脉粥样硬化性心脏病(冠心病)冠状动脉病变程度的关系。方法选取在武汉大学人民医院心内科住院行经皮冠状动脉介入(percutaneous coronary intervention,PCI)治疗的254例冠心病患者,按照红细胞分布宽度(red blood cell distribution width,RDW)中位数13.0%,分为高RDW水平组(≥13.0%)(n=135)和低RDW水平组(<13.0%)(n=119),比较两组患者临床资料及冠状动脉病变情况;采用多元逐步回归分析冠状动脉病变支数与RDW的相关性。结果与低RDW组比较,高RDW组心肌梗死患者比例更高、冠状动脉病变支数更多、病变程度更重,差异有统计学意义(P<0.05)。随着冠状动脉病变支数的增加,RDW也随之增加,RDW与Gensini积分存在正相关(r=0.468,P<0.001),线性相关性良好,并且随着冠状动脉病变程度的升高(Gensini积分增加)而升高。多元逐步回归分析显示,RDW与冠状动脉病变程度独立相关(β=0.242,P=0)。结论 RDW与冠心病冠状动脉狭窄程度有关,可以作为冠状动脉病变程度的评价指标之一,但需综合多方面的因素,理性评估其临床意义。Objectives To explore the relationship of red blood cell distribution width (RDW) and severity of coronary artery disease. Methods A total of 254 consecutive patients with coronary artery disease received percutaneous coronary intervention (PCI) in People's Hospital of Wuhan University were retrospectively studied. According to the RDW median 13.0%, the patients were divided into high level of RDW group (13.0%) and lower level of RDW group (〈13.0%). Clinical data and patients' characteristics of coronary artery disease between the two groups were compared. Association of coronary artery lesions and RDW was analyzed by using multiple stepwise regression analysis. Results Higher percentage of patients with myocardial infarction, more counts of coronary artery lesions, severer degree of coronary artery lesions were in high level of RDW group than in lower level of RDW group (P〈0.05). As coronary artery lesion counts increased, RDW level also increased. And RDW was positively correlated with Gensini score (r= 0.468, P〈0.001 ), the linear correlation was good. RDW increased with the severity of coronary artery lesion level (Gensini score increased). Multiple stepwise regression analysis showed that RDW was independently associated with the degree of coronary artery lesions (β=0.242,P=0). Conclusions RDW is associated with the degree of coronary artery stenosis and might be used as one of the evaluation indices of severity of coronary heart disease, when considering other possible relevant factors.
