腹腔局部细胞因子和子宫内膜异位症、子宫腺肌病致不孕相关性

摘　　要：目的:分析腹腔局部细胞因子和子宫内膜异位症、子宫腺肌病至不孕相关性,为临床诊治提供参考。方法:选取我院子宫内膜异位症37例、子宫腺肌病致不孕患者20例为研究组,另外选取同期健康体检妇女为对照组,研究组患者行腹腔镜手术治疗,测定所有人员腹腔液干扰素、白细胞介素和肿瘤坏死因子等因子,并分析腹腔细胞因素与子宫内膜异位症分期相关性。结果:子宫内膜异位症患者IFN-r、TNF-α、白细胞介素以及VEGF水平均显著高于对照组和子宫腺肌病组,P<0.05,子宫腺肌病组患者IL-10水平(15.23±4.92)pg/ml显著高于对照组(7.82±3.26)pg/ml,P<0.05。经person相关系数分析,内膜异位症患者腹腔液中TNF-α与IL-6(r=0.567)、IL-10(r=0.791)、IL-17(r=0.592)呈现正相关,P<0.05。结论:子宫内膜异位症和子宫下疾病患者盆腔局部细胞因子表达异常,可能干扰正常生殖过程引发不孕。Objective： To analysize the correlation of Intraperitoneal local cytokine with endometriosis, adenomyosiscause infertility,and provide reference for clinical Diagnosis and Treatment.Methods：37 cases endometriosis and 20 casesadenomyosis cause of infertility patients in hospital were selected,Healthy women over the same period were selected ascontrol group,study group patients were taken trentment of Laparoscopic surgery,Peritoneal fluid interferons, interleukinsand tumor necrosis factor and other factors of patients were compared,peritoneal cells factors and endometriosis staging wasanalyzed.Results：Endometriosis patients IFN-r, TNF-α, interleukin-and VEGF levels were significantly higher than controlgroup and adenomyosis group, P〈0.05. Adenomyosis patients IL-10 levels （15.23 ± 4.92） pg / ml were significantly higherthan control group IIL-10 levels （7.82 ± 3.26） pg / ml, P〈0.05.By pearson correlation analysis, patients with endometriosisperitoneal fluid TNF-α and IL-6 （r = 0.567）, IL-10 （r = 0.791）, IL-17 （r = 0.592） were showed a positive correlation, P〈0.05.Conclusion： Endometriosis and pelvic disease in patients with uterine abnormalities local cytokine expression, may interferewith the normal reproductive process caused infertility.
