右美托咪定用于预防全麻非体外循环冠状动脉旁路移植术患者术后寒战随机对照研究

摘　　要：目的:观察全麻非体外循环不停跳冠状动脉旁路移植术(CABG)患者术后应用右美托咪定镇静对术后寒战的预防作用。方法:选取实施全麻非体外循环不停跳CABG手术的患者60例,随机分为观察组(术后应用丙泊酚联合右美托咪定镇静)和对照组(术后单纯使用丙泊酚镇静)各30例。观察术后寒战发生率、寒战分级及全麻清醒过程中(停用丙泊酚后)呼吸循环指标的波动;比较观察组与对照组脱离呼吸机过程的区别。结果:观察组术后寒战发生率13.3%,对照组发生率46.7%,两组差异有统计学意义,且对照组发生寒战的严重程度较高;两组患者术后监测的呼吸循环指标对比差异均无统计学意义。结论:全麻非体外循环不停跳CABG患者于术后应用右美托咪定静脉泵入可有效预防寒战发生,且对呼吸循环指标无影响。Objective:To observe efficacy of dexmedetomidine in preventing shivering after general anesthesia for CABG surgery.Methods:Sixty patients scheduled for off-pump CABG surgery were randomized into dexmedetomidine group(calm with propofol and dexmedetomidine,n=30)and control group(calm with propofol,n=30).Observed the incidence of postoperative shivering and the classify grade of shivering.Observed the undulate of respiratory cycle index.Showed the difference between groups.Results:The patients in the control group showed a significantly higher postoperative incidence of shivering(46.7%)than those in dexmedetomidine group(13.3%).The difference between groups on undulate of Respiratory cycle index had not statistically significant.Conclusion:Dexmedetomidine can lower the incidence of shivering after general anesthesia for CABG surgery and has no effect on respiratory cycle index.
