摘 要:目的了解河南省中医系统医院感染变化趋势,为优化医院感染控制措施提供决策依据。方法 2013-2015年连续三年在河南省60所中医院开展医院感染现患率调查,借助全国医院感染监控管理培训基地网上办公系统收集数据,分析三年医院感染变化趋势。结果 60所中医院2013-2015年三年应查人数分别为18 112例、19 135例、19 352例,实查人数分别为18031例、19045例、19161例,实查率分别为99.55%、99.52%、99.01%;三年医院感染现患率分别为2.21%、1.57%、1.72%,医院感染现患率总体呈下降趋势;三年医院感染现患率均居前十的科室有7个,分别是综合ICU、血液病科(组)、肿瘤科、神经外科、神经内科(组)、普外科、泌尿外科,其中血液病科(组)、神经外科、神经内科(组)、普外科下降差异有统计学意义;三年Ⅰ类手术切口患者手术部位医院感染现患率分别为0.74%、1.05%、0.81%,呼吸道感染占医院感染首位。结论 2013-2015年60所中医院医院感染现患率总体呈下降趋势,综合ICU依然为医院感染高危科室;而血液病科(组)、神经外科、神经内科(组)、普外科医院感染管理工作取得明显成效,同时对于肿瘤科、泌尿外科需要进一步加强医院感染管理力度。OBJECTIVE To investigate the prevalence of nosocomial infections in traditional Chinese medicine hospi- tals in Henan province so as to provide guidance for optimization of control measures for the nosocomial infections. METHODS The prevalence rates of nosocomial infections were investigated in 60 traditional Chinese medicine hos- pitals in Henan province from 2013 to 2015, then the data were collected by means of online office system of na- tional nosocomial infection monitoring and management training base, and the change of incidence of nosocomial infections in the three years was analyzed. RESULTS Totally 18 112 patients who were hospitalized in the 60 hospi- tals were supposed to be investigated in 2013, 19 135 patients in 2014, 19 352 patients in 2015; totally 18031 pa- tients were actually investigated in 2013, 19045 patients in 2014, 19161 patients in 2015% the actual investigation rate was 99. 55% in 2013, 99. 52% in 2014, 99.01% in 2015; the prevalence rate of nosocomial infections was 2.21% in 2013, 1.57% in 2014, 1.72% in 2015, showing a downward trend. Seven departments, including com- prehensive ICU, blood disease department (group), oncology department, neurosurgery department, neurology department (group), general surgery department, and urology department, ranked the top 10 prevalence rate of nosocomial infections in the three years; there was significant difference in the decrease of prevalence rate of noso- comial infections among the blood disease department (group), neurosurgery department, neurology department (group), and general surgery department. The prevalence rate of surgical site infections in the patients undergoing type I incision surgery was 0.74% in 2013, 1.05% inn 2014, 0.81% in 2015; the respiratory tract infections ranked the first place. CONCLUSION The prevalence rate of nosoeomial infection shows a downward trend in the 60 traditional Chinese medicine hospitals from 2013 to 2015, and the comprehensive ICU remains the department at high risk of nosocomial in
