摘 要:目的探讨水胶体敷料+PVP碘治疗经外周中心静脉穿刺(PICC)置管穿刺部位感染患者的效果。方法选取2014年9月-2015年12月医院PICC置管穿刺部位感染患者216例,依据随机数表法分为观察组和对照组,比较两组患者的临床疗效、疼痛分级及红肿消退时间、感染治愈时间。结果观察组总有效率为98.15%;对照组总有效率为76.85%,差异有统计学意义(P<0.05),观察组患者疼痛分级:0级90例占83.33%,1级13例占12.04%,2级及以上5例占4.63%;对照组患者疼痛分级:0级66例占61.11%,1级29例占26.85%,2级及以上13例占12.04%,差异有统计学意义(P<0.05);观察组患者红肿消退时间、感染治愈时间分别为(2.13±0.52)d、(8.15±2.04)d;对照组患者红肿消退时间、感染治愈时间分别为(3.67±0.74)d、(13.96±2.45)d,差异有统计学意义(P<0.05)。结论水胶体敷料联合PVP碘治疗PICC置管穿刺部位感染成效显著,能够明显改善患者红肿、疼痛等临床症状,值得临床推广。OBJECTIVE To explore the effect of hydrocolloid dressing plus PVP iodine on treatment of patients with puncture site infections of peripherally inserted central catheter (PICC). METHODS A total of 216 patients with PICC puncture site infections who were treated in hospitals from Sep 2014 to Dec 2015 were enrolled in the study and randomly divided into the observation group and the control group. The clinical efficacy, pain grade, swelling subsided time, and infection healing time were observed and compared between the two groups of patients. RESULTS The total effective rate was 98.15% in the observation group, 76.85% in the control group, and there was significant difference (P〈0.05). In the observation group, 90 (83.33%) patients had grade 0 pain, 13 (12.04%) had grade 1 pain, and 5 (4.63%) had grade 2 or above pain. In the control group, 66 (61.11%) had grade 0 pain, 29 (26.85%) had grade 1 pain, and 13 (12.04%) had grade 2 or above pain; there was significant difference(P〈0.05). The swelling subsided time and infection healing time of the observation group were (2.13 ± 0.52) d and (8.15 ± 2. 04) d, respectively ; the swelling subsided time and infection healing time of the control group were (3.67±0.74)d and (13.96±2.45)d, respectively, and there was significant difference (P〈0.05). CONCLUSION The hydrocolloid dressing plus PVP iodine can achieve significant effect on treatment of the PICC puncture site infection and remarkably improve the clinical symptoms such as swelling and pain. It is worthy to be promoted in the hospital.
