“锚定法”改良颈后路单开门椎管扩大成形术的临床研究及应用

摘　　要：目的:针对改良颈后路单开门椎管扩大成形术在临床当中对多节段脊髓型颈椎病进行治疗所起到的效果进行研究。方法:随机在2013年2月-2014年2月期间于本院就诊的多节段脊髓型颈椎病患者中选取25例作为研究组,并将该25例患者治疗之前的生理状况作为对照组,所有患者行"锚定法"(即改良颈后路单开门椎管扩大成形术)。两年随访,对比分析治疗前后25例患者的ROM值、屈曲角度以及后伸角度、JOA分值、神经功能改善情况以及患者并发症发生率。结果:经过手术干预,两年随访期间,25例患者当中均无明显并发症状,仅有2例患者发生轻微的创口感染;另外,在ROM值、屈曲角度以及后伸角度、JOA分值以及神经功能改善情况上均呈现了明显的好转,治疗前后比较差异均有统计学意义(P<0.05)。结论:在对多节段脊髓型颈椎病进行治疗的过程当中,行以改良颈后路单开门椎管扩大成形术("锚定法")能够获得比较理想的临床效果,有较高的临床实用价值。Objective： To study the effect according to the modified neck after expansive open-door vertebral canal expansion forming operation in clinic for multi segmental cervical spondylotic myelopathy. Method： A total of 25 cases with multilevel cervical myelopathy in our hospital were selected as the study group from February 2013 to February 2014, and the physical condition before of them were selected as control group. All patients were treated with ＂anchor method＂ （ modified posterior open-door laminoplasty ） .After two-year follow-up, the ROM values, flexion and after extension angle, JOA score, neurological function improvement and the complications rates of 25 patients before and after treatment were analyzed and compared.Result： After surgical intervention, during two years follow-up, 25 patients were not obvious complications occurred, only 2 patients occurred slight wound infection.In addition, before and after treatment, the ROM values, flexion and after extension angle, JOA score and the improvement of neurological function showed significant improvement, the differences were statistically significant （P〈0.05） .Conclusion： In the process treatment of multi segmental cervical spondylotic myelopathy, to modified neck after laminoplasty forming operation（the ＂anchor method＂ ） can obtain satisfactory clinical effect and has high value of clinical application.
