摘 要:目的评价^(18)F-FDG PET/CT显像对心脏肿瘤的良、恶性鉴别诊断价值。资料与方法对连续20例行PET/CT检查发现有心脏或心包肿瘤患者的临床及影像资料进行回顾性分析。将PET/CT诊断结果与最终临床诊断对照,评价其对心脏良、恶性病变的鉴别诊断价值。结果 PET/CT共检出心脏单发病变6例,其中4例为良性;心脏病变伴全身多发病变14例,其中13例为恶性病变。心脏良性病变的SUVmax为0.5~7.6,平均3.4±3.0;恶性病变的SUVmax为2.0~21.9,平均7.5±5.6。PET/CT对心脏良、恶性肿瘤诊断的灵敏度为100%,特异度为80.0%,准确度为95.0%。结论 ^(18)F-FDG PET/CT显像对心脏肿瘤的良、恶性鉴别诊断具有重要价值,病变FDG摄取与全身分布情况及临床资料结合具有较高的诊断准确性。Purpose To evaluate the clinical value of ^(18)F-FDG PET/CT in differentiating benign lesions from malignant ones in patients with cardiac tumor. Materials and Methods The ^(18)F-FDG PET/CT data of 27 consecutive patients detected with cardiac or pericardiac lesions were retrospectively analyzed. The diagnostic findings of ^(18)F-FDG PET/CT were compared with the final clinical results to assess its diagnostic value for identifying malignancies or benign lesions. Results ^(18)F-FDG PET/CT found single cardiac lesions in 8 cases, among which 6 were benign; it also detected cardiac lesion associated with systemic multiple lesions in 19 cases, among which 18 were malignant. The average maximum standardized uptake value in malignant lesions(2.0-21.9, 7.5±5.6) was significantly higher than that in benign lesions(0.5-7.6, 3.4±3.0). According to the current diagnostic criteria, the sensitivity of differential diagnostic value of ^(18)F-FDG PET/CT reached 100%, the specificity was 80.0% and the accuracy was 95.0%. Conclusion 18 F-FDG PET/CT plays an important role in the differentiation of benign and malignant lesions in cardiac tumors. The combination of FDG uptake in lesions and other clinical examinations will bring a better accuracy in diagnosis.
