经皮微创手术联合中药熏洗治疗闭合性跟腱断裂的疗效观察

摘　　要：目的：观察经皮微创手术配合中药熏洗治疗闭合性跟腱断裂的临床疗效。方法新鲜闭合性跟腱断裂患者60例，随机分为2组，经皮微创＋中药熏洗组30例，微创经皮手术配合温水泡洗组30例，治疗后观察跟腱愈合、功能评分及相关并发症的发生情况。结果60例患者均获得随访，随访时间6~10个月，平均8.6个月。彩超检查提示跟腱均愈合，愈合时间9~12周，平均10.5周。所有患者均无切口感染、跟腱术后再断裂、跟腱粘连、跟腱挛缩、中药过敏等情况发生。根据美国足踝外科AOFAS评分标准，中药熏洗组，优21例、良5例、可4例，优良率86.7%；温水泡洗组，优15例、良7例、可8例，优良率73.3%。两组患者AOFAS评分比较，差异有统计学意义（P〈0.05）。结论中药熏洗配合经皮微创手术治疗新鲜的闭合性跟腱断裂具有操作简单、并发症少，跟腱愈合率高等优点，值得临床推广。Objective To explore the clinical effect of treatment with percutaneous minimally invasive surgery combined with herbal fumigation for acute closed rupture of Achilles tendon.Methods Sixty cases of closed rupture of Achilles tendon were treated randomly from June 2012 to February 2015. In the Herbal fumigation group,There were 23 men and 7 women,aged from 23~49years old,with a mean age of 38.2 years;These cases were treated by percutaneous minimally invasive surgery combined with herbal fumigation. In the control group,They were 22 men and 8 women,aged from 20~46years old,with a mean age of 37.6 years. These cases were treated by percutaneous minimally invasive surgery combined with hot water fumigation. Results All of these patients were followed up with 6 to 10 months,they were recovered completely. The function was evaluated by the AOFAS scales,excellent and good rate of was86.7%in the herbal fumigation group,while in the control the excellent and good rate was 73.3%. The difference between the two groups was statistically significant（P〈0.05）. These complications included infection;rupture again;achilles tendon adhesions,achilles tendon contractures;herbal allergy did not occur. Conclusion Percutaneous minimally invasive surgery combined with herbal fumigation is a good means of treatment for acute closed rupture of achines tendon,as it is percutaneous minimally invasive;beautiful appearance and simple,moreover,this technology is easy to learn and easy to grasp.
