抗骨质疏松联合股骨近端锁定钢板治疗高龄粗隆间骨折疗效分析  被引量:1

摘  要:目的观察抗骨质疏松联合股骨近端锁定铜板内固定治疗高龄股骨粗隆间骨折的临床疗效。方法回顾性分析89例高龄股骨粗隆间骨折患者应用抗骨质疏松联合股骨近端锁定钢板内固定治疗的临床资料,术后观察骨折愈合、再骨折情况,分别术前、术后测量健侧髋关节骨密度及术后进行髋关节Harris评分等。结果86例患者获得20~30个月随访,术后6、12、18个月测得的健侧髋部平均骨密度T值与术前健侧平均骨密度T值比较差异有统计学意义(P〈0.01);1例发生骨折延迟愈合,3例发生健侧髋部再骨折;术后12个月、18个月髋关节功能Harris评分优良率分别为91.9%分和90.7%。结论抗骨质疏松联合股骨近端锁定钢板内固定可有效改善高龄粗隆间骨折患者骨密度,有利骨折愈合,避免骨质疏松加重和再骨折,髋关节功能Harris评分优良率高。、Objective To observe the clinical effect of anti osteoporosis combined with proximal femoral locking plate fixation in the treatment of femoral intertrochanteric fracture in elderly patients. Methods 89 cases of femoral coarse intertrochanteric fractures were treated with anti osteoporosis combined with femoral proximal locking plate internal fixation for the treatment of clinical data, postoperative observation of fracture healing, and fracture, respectively the preoperative and postoperative measurement of health after the hip bone mineral density and operation Harris graded. Results 86 cases had 20-30 months follow-up, in postoperative 6 months, 12 months, 18 months, health side hip average bone densitywas measured, tvalue of the difference was statistically significant between the t value mean BMD of these time points onhealth side preoperatively ( P〈0.001 ) ; 1 case of fracture delayed union, healthy contralateral hip fracture occurredin 3 cases; the excellent and good rate 12 months and 18 months hip Harris score were91.9% and 90.7% respectively. Conclusion The anti osteoporosis combined with femoral proximal locking plate internal fixation can effectively improve the old coarse intertrochanteric fracture patients with bone mineral density, improve the fracture healing, to avoid osteoporosis exacerbation and re fracture, Harris hip function score is excellent and good rate ishigh.
