高压注射碘对比剂渗漏预防及护理

摘　　要：目的分析高压注射碘对比剂渗漏的原因，寻找减少渗漏的措施，观察分析中西医结合对碘对比剂渗漏的护理效果。方法回顾分析12825例增强CT、CTA与下肢深静脉造影检查患者中15例渗漏患者〈24h用50％硫酸镁湿敷加冰袋冷敷，〉24h用中成药止痛消炎软膏外敷加热水袋热敷。结果对比剂速率、温度、注射静脉好坏、护士注射技术及患者情绪精神状态心理状况等均影响对比剂的渗漏；中西医结合护理对渗漏有加快康复的效果。结论注射前必须有全面合理的措施才能减少渗漏的发生。Objective To analyze the causes of extravasation with high pressure injection of iodine contrast agent, and to find the measures to reduce the leakage. Methods The clinical data of 12825 patients with enhanced CT, CTA and deep venous angiography were retrospectively analyzed, including 15 cases of Iodine contrast agent extravasation. Results Contrast agent injection rate, temperature, situation of vein, nurse injection technique and emotional state of the patients can affect the contrast agent extravasation.15 patients with leakage were treated with wet compress with 50% magnesium sulfate and cold compress with ice packs for 24 hs.After 24-48 hs, Chinese medicine was used to relieve pain and eliminate inflammation and ointment is also used.meanwhile, these patients were heated with hot packs. Conclusion It is necessary to have a comprehensive and reasonable measures to reduce the occurrence of Iodine contrast agent extravasation, and combination of traditional Chinese and Western medicine can make the patients of contrast agent extravasation recover faster.
